About the job

  • Provide a 2-hour rapid response for patients who are entering a health crisis across all of Dorset
  • Provide advanced levels of assessment to investigate, diagnose and treat patients in their own environment and prevent unnecessary hospital admissions.
  • Supporting clinical triage of patients on the 111 CAS and co-ordinating visits to UCR car
  • Completing urgent joint visits providing therapy expertise to the assessment, intervention, and the provision of equipment and onwards referrals to community teams
  • Work in collaboration with other members of the MDT
  • Work flexibly in response to service need across multiple sites and areas.
  • Directly working with 111/999 and local community teams and Primary Care Networks.
  • Opportunity to rotate between services
  • Support the development of 24-48 follow up

Main duties of the job

You will be a key member of our UCR service working in a holistic multidisciplinary team including, Specialist Nurse/Paramedic Practitioners, Senior Physiotherapists/Occupational Therapists, Nurses, Paramedics and Assistant Practitioners. The service provides a 2-hour response to patients who are experiencing a health crisis with the aim to support them in their own homes (including care homes) rather than being admitted to hospital.

A high level of skill and autonomous decision making is required when assessing, caring and managing the risk for unwell patients in their homes and supporting families and carers. You will work closely with   multi- disciplinary colleagues and provide clinical support and guidance for staff.

The post holder will be required to liaise with their team colleagues in the other localities covered by the service to ensure cross cover can be provided according to demands on the service.

The team works to prevent avoidable hospital admissions, and refers onto local cluster hubs for on-going intervention to support the patient remaining in their own home.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Educational Requirements / Qualifications

Essential criteria
  • Registered Allied Health Professional(AHP on HCPC register)
  • BSc (Hons)/Diploma in Physiotherapyor Occupational Therapy
Desirable criteria
  • Non-medical prescribing qualification
  • Additional post-registration qualifications

Skills / experience / knowledge

Essential criteria
  • Recent previous experience within a comparable role
  • Experience ofworking autonomously at an experiencedlevel within relevant service (eg. Rapid response/ acute/emergency community or urgent care) delivering effective patient focused care
  • Previous clinical experience in the community and/or acute setting with adults and older people
  • Knowledge of long term conditions such as; Orthopaedicconditions, Heart Failure, COPD, falls, trauma and/or neurology experience
Desirable criteria
  • Experience of supervision of staff
  • Previous experience of working in the community
  • Experience of using the Single Assessment process
  • Experience of operating within a health professional duty service
