An opportunity has arisen for a Band 6 Mental Health Practitioner to join the Memory Assessment Team in Caerphilly.
We are seeking a candidate who is who is highly innovative, enthusiastic, and passionate about working with older people in a person-centred approach. The role requires the successful candidate to assess and support people with a diagnosis of cognitive impairment, dementia, and their families.
The successful candidate will be somebody with passion for delivering high quality patient centred care, in a forward thinking and developing memory service.
The post entails delivering memory assessments, balancing risk, and dealing with patients with complex needs within a friendly and supportive team
Main duties of the job
We are looking for a Mental Health Practitioner who is highly innovative, enthusiastic, and passionate about working with older people in a person-centred approach. The role requires the successful candidate to assess and support people with a diagnosis of cognitive impairment, dementia, and their families.
This is an exciting opportunity to join a friendly and supportive team in a large area that has a growing population of older people. You will have knowledge and experience in the care of people with cognitive and enduring mental health problems in a community setting. You will have the opportunity to work alongside other professionals within the MDT.
You will be committed to ensuring the delivery of the highest quality user-centred care and contributing to the development of mental health services in the Borough of Caerphilly.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The post holder will be working across the Older Adult Memory Assessment Service . Facilitating and co-ordinating the timely and effective assessment / discharge / transfer of patients by utilising clinical, managerial and communication skills. The post holder will be responsible for the timely assessment and management of patients who have complex health and social needs as a result of memory difficulty.
The post holder will play a pivotal role in liaising with Primary Health to enable the team to provide early specialist input into assessment, advice and support on a range of mental health problems. The post holder will offer specialist mental health advice and assessment in addition to raising standards in awareness of and treatment in mental health difficulties in older adults.
The post holder will ensure service delivery is of a high professional standard that is in line with Health Board policies and procedures.
