Older Adult Services

Community Mental Health Social Worker (Flow)

Countywide role based at one of the following bases Stratford Healthcare, Longford Clinic Coventry, Rugby, Nuneaton (TBC)

We are developing a new team to work with Older Adults who have mental health needs in the Community and following an in-patient stay.

The service is being developed in conjunction with health, social care, statutory and voluntary sector partners to have an integrated service with partnership working.

Our new team is taking shape, and we want this to be an integrated multi-agency and multi-disciplinary service.  We are currently proposing a model of OT, Nursing, Psychiatry, Psychology, Social Worker, Voluntary sector, support workers and peer workers and will work closely with the Dementia Service.

As a social worker in the team, we will ensure that you have clinical and professional support from a qualified Social Worker to support the maintenance of your professional registration. This support will be alongside the supervision and management that will come from the Trust giving the best of both worlds.

The successful applicant will work alongside the leads and colleagues to continue to develop the service and begin to pick up a specific relevant clinical caseload, and provide evidence based interventions as well as liaising with other agencies.

We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player.  Good communication and problem-solving skills are essential, as well as knowledge of mental illness and forensic awareness.    We also are looking for people who can demonstrate the Trust’s vision and values in practice, and are motivated to be involved in innovation and service development.

  • Relevant Professional Qualification ie. RMN, DipCOT
  • Current unrestricted registration with HCPC/NMC
  • Specialist knowledge across a range of procedures and theory
  • Professional knowledge acquired through degree/ diploma
  • Relevant training and experience specialist short courses
  • Evidence of continuing professional development

EXPERIENCE

  • Substantial experience in a Community Mental health care setting.
  • Developed specialist knowledge gained within a similar environment over a considerable period of time
  • Significant experience of undertaking assessment, formulation, risk assessment and positive risk management
  • Experience of safeguarding and investigations
  • Proven experience of people management
