We have an exciting opportunity for someone to join our Cancer Screening Team 

12 month Secondment (37.5 hours a week)  

The role of a Cancer Screening Liaison Support Worker (CSLSW) has been developed specifically to provide support to individuals who may be high risk or experiencing Cancer diagnosis. They will offer empathy and understanding to others, inspiring hope and recovery. The CSLSW will work alongside practical support networks, in order to help people feel in control of their lives and identify opportunities to support individuals to engage with cancer screening programmes of care.

To encourage, inspire, motivate and assist people who access CWPT services with multiple and complex needs. Working alongside the Practitioner, you will assist in carrying out a comprehensive and holistic assessment and developing a package of care and support to promote the person’s ability to engage with appropriate services, including cancer screening.

The CSLSW will work as part of a team that provides services to vulnerable people with complex needs in a community setting, focusing on the direct needs of the individuals and working across boundaries of care, organisations and roles.

To build links with, and liaise with external agencies to work together in a collaborative approach to meet the needs of the person accessing services.

To have the individual’s needs to the fore at all times, working to the recovery model to support them to work towards their identified goals.
