•    Primary and secondary mental health care for children and adults including specialist inpatient child and adolescent mental health provision, perinatal mental health, forensic services including low and medium secure care.
•    Specialist community support for children and adults with learning disabilities and autism, including intensive support.
•    Community physical health and well-being services for children and adults.

The Trust was first established in 2002 and employs approximately 7,000 staff who provide care from more than 400 sites. An exciting opportunity has arisen for a dynamic Mental Health Nurse to join the Positive Lifestyle Team on a full time basis.

 

Prif ddyletswyddau'r swydd

The successful applicant will work across the Greater Preston and Chorley/South Ribble Clinical Commissioning Group footprint.  The role will involve working with Service Users who have a Severe Mental Illness, referred from the CMHT’s requiring interventions around Physical Health Monitoring and improving their quality of life, as well as monitoring their mental health.  It is anticipated that the successful applicant will have a caseload under supervision.

 

 

Gweithio i'n sefydliad

The successful applicant will be able to work well within a team as well as independently and have excellent liaison and communication skills. Working across both the Primary Care and Mental Health Services means they will need to be flexible and adaptable and able to form good working relationships with others. 

 Regular supervision and CPD opportunities are available.
