Employing almost 7,500 colleagues, affectionately known as Team LGT, we provide services at Queen Elizabeth Hospital in Woolwich, University Hospital Lewisham, and at over a dozen community settings in Lewisham. We also provide some services at Queen Mary’s Hospital in Sidcup.

Our hospitals and community sites provide a wide range of inpatient and outpatient services, as well as emergency and planned care. The Trust is rated as “good” or “outstanding” in over three-quarters of the services inspected by the Care Quality Commission.
Every year our work includes performing 10,000 procedures in our theatres; bringing seven and a half thousand new lives into the world; carrying out 570,000 visits to patients in their homes or communities and providing emergency care for more than 300,000 patients arriving in our busy Urgent and Emergency Departments.
LGT is a centre for the education and training of medical students enrolled with King’s College London’s GKT School of Medical Education. We are a training centre for nurses, midwives and allied health professionals. We are pioneering new roles that will support the changing needs of our patients and are one of the largest employers of physician associates in the country.

This role is available across two Primary Care Networks (PCNs) in Lewisham:
- Modality Primary Care Network
- Aplos Primary Care Network
Successful candidates will be employed by one of the above PCNs and aligned to a neighbourhood-based INT. Please indicate your preferred PCN(s) or neighbourhood(s) in your application. Final placement will be based on service need and candidate suitability.
Main duties of the job
Are you passionate about making healthcare more inclusive and community-centred? We are looking for Community Link Workers to join the newly launched Integrated Neighbourhood Teams (INTs) in Lewisham, with roles available across three Primary Care Networks (PCNs).
This role will be vital in building trust between local communities and healthcare services, particularly for residents facing barriers to care. You’ll work closely with faith groups, voluntary and community organisations (VCSEs), and health teams to strengthen access, reduce inequalities, and champion health equity across Lewisham.

We focus on accessible, high-quality services, and personalised health support. As part of the Modality Partnership, we uphold NHS values while improving outcomes for our diverse local community.
Aplos Health PCN is a collaborative network of four GP practices in south and west Lewisham, serving over 50,000 patients. We deliver integrated, community-focused care, addressing health inequalities through innovative services like youth wellbeing clinics and digital hubs. Our multidisciplinary team is committed to improving health outcomes and empowering local residents.

Main Responsibilities
- Act as a bridge between the community and primary care to improve access and build trust.
- Engage with local VCSEs, faith groups, and community leaders.
- Support residents with access barriers (e.g., language, registration, social challenges).
- Coordinate outreach events like pop-up clinics and health promotion activities.
- Build partnerships to co-design solutions to health inequalities.
- Collect feedback and share insights to influence service development.
