Job overview

As part of the expansion of our Community services development, we are looking to recruit a Band 4 Community Rehab Practitioner to join the Intermediate care team in our Community hub in Guildford and Waverley. We would welcome applicants who are interested in developing their skills as part of a role development program.

Role summary

We are passionate about supporting patients at home, preventing unnecessary hospital admissions and supporting people when they are discharged from hospital. You will become part of our team working with Occupational Therapists, Physiotherapists and Nurses to provide short term care and rehabilitation to patients in their own homes to help them regain their independence. You will deliver care to patients whilst also supporting their rehabilitation programme as overseen by the registered staff. You will also work alongside the nursing lead for the team to assist with specific tasks as required.

 The role also supports with the allocation of patient visits and provides guidance for band 3 colleagues while out on visits in conjunction with the team lead.

You will be supported with training and career development opportunities which includes a Nationally recognised care certificate and local team competency programme. You will have the opportunity to develop your expertise in a community setting with patients with a range of conditions.

Main duties of the job

JOB PURPOSE

-To work as part of the Intermediate Care team which provides short term support to people being discharged from hospital or to keep them in their own home, the team work with our patients to enable them to achieve their optimum independence 

Patients will require support with personal care, mobility, medication administration and exercise programs to achieve their individual recovery and rehabilitation goals .You will take on new patients on the Intermediate Care Team caseload and help with the planning of their care, supported by our nurses and therapists if required for complex clinical decision making.

The post holder will be key to supporting the wider team and ensuring visits are all completed on the day, helping with allocation, scheduling and adjustments throughout the day

Detailed job description and main responsibilities

-To provide Therapy assessment and treatment of patients referred to the service designated by a qualified Occupational Therapist/ Physiotherapist or nurse

- Assesses and treats patients without direct supervision according to team/department standards and protocols

-Ability to manage own caseload/prioritization

-To act in a mentorship role for band 3 CRAs for Intermediate care

-To complete patient new take on to Intermediate care as a delegated take once the referral has been triaged by a registered healthcare professional

-To  complete patient allocation of visits for Intermediate care

- May be a rotatory and flexible post working across Intermediate care service and our Urgent community response service .The main role will sit in intermediate care (D2A)

- Working flexibly within a 7day service.

Hours: 7 day service-8am-8pm

Qualifications

Essential criteria
  • NVQ 3 level 3 equivalent of knowledge and experience
  • Portfolio evidence of relevant experience/ CPD in specialist field.

Knowledge

Essential criteria
  • Able to work on own initiative and recognise when to seek assistance
  • Broad experience of working as a Senior Community rehabilitation Assistant with some experience supporting more junior staff or relevant Carer experience
  • NHS experience within acute &/or community setting
  • Recent experience of working as part of a multidisciplinary team, with an understanding of roles and responsibilities of the team.
  • Understanding of common medical conditions and terminology
  • Able to perform a basic primary assessment according to unit guidelines
