The Patient Safety Case Managers are clinical investigators with the responsibility for overseeing the management of cases triggered as part of the Trusts Patient Safety Incident Response Plan (PSIRP).

the post holder will work closely with clinical teams ensuring that patient safety incidents are rapidly and comprehensivley reviewed to identify learning and improvement opportunities as well initiate futher investigation in line with the Trusts PSIRP.  

Prif ddyletswyddau'r swydd

•    The post holder will support the clinical triaging of patient safety events and where required, the initiation of further investigation. This review will be clinically complex and will require in depth review of clinical records to establish a narrative describing the events relating to the patient safety event, which will formulate a key component of the Initial Incident Review (IIR) undertaken by the clinical speciality concerned. The post holder will maintain oversight of the completion of the Initial Incident Review. Compassionate engagement with those affected by patient safety event will also be managed and supported by this post holder. 
•    The post holder will liaise with the appropriate teams to further manage the response to the patient safety event. 
•    The post holder will be responsible for ensuring all documentation/reports relating to PSIRF responses is appropriately completed by all individuals affected by the event. The post holder will also be expected to lead and facilitate After Action Reviews (AAR).
•    The post holder will be highly skilled in the development of patient safety investigation reports and will be expected to take the lead on Trust wide safety improvement activities. 
•    The post holder will have broad clinical knowledge and experience and will use their reasoning, clinical acumen to manage their workload, make decisions and proactively deal with issues independently.

Experience

  • Experience of working as a senior manager in the NHS
  • In-depth knowledge of healthcare risk management policy in the NHS; NHS risk management principles; and NHS (Datix) incident reporting system
  • Substantial experience of patient safety and safety improvement

Skills/ Knowledge/ Abilities

  • In-depth knowledge and understanding of patient safety and safety governance frameworks
  • Skilled in the analysis and interpretation of patient safety data
  • Ability to motivate, empower and facilitate teams from different professional disciplines to be proactive in patient safety governance and safety improvement
  • Demonstrable understanding of the NHS regulatory and compliance frameworks under which NHS Trusts operate
  • Good understanding of complex issues within healthcare organisations and knowledge of recent policy developments
  • Theoretical and practical knowledge through experience of risk, governance, patient safety
  • Able to demonstrate awareness of the professional environment, and understanding of legislation and accountability frameworks.
  • Excellent communication, negotiation and influencing skills.
  • Good interpersonal skills with proven ability to communicate clinically complex, sensitive and contentious information to patients/carers and staff.
  • Able to maintain high standards of diplomacy and confidentiality
  • Ability to establish credibility and maintain effective working relationships with multidisciplinary colleagues
  • Proven communication skills both orally and in writing.
  • Experience of the management of information systems and the development of databases or spreadsheets, ability to take and transcribe minutes.
  • Able to plan and implement patient safety related projects across multiple services.
