Job overview

Hounslow PCNs are looking for a first contact paramedic to join Feltham and Bedfont PCN and to work in collaboration with the PCN Lead Paramedic and other PCN Paramedics across Hounslow. Our growing multi-disciplinary PCN teams working across the five PCNs in Hounslow: Brentworth, Chiswick, Feltham & Bedfont, Great West Road and Hounslow Health. 

Hounslow is a diverse outer London Borough of approximately 324,000 registered patients. Within Feltham and Bedfont the PCN Paramedic service supports 13 GP Practices. The Hounslow Consortium brings together the borough's five Primary Care Networks (PCNs) to work collaboratively for the benefit of patients and their practices. 

We are seeking individuals who are passionate about improving care for patients living in areas of high socioeconomic deprivation, tackling health inequalities and increasing access to high quality multi-disciplinary general practice.

Main duties of the job

The role will give you a solid grounding in Primary Care, working in collaboration with your PCN Lead Paramedic and across Feltham and Bedfont PCN, including alongside other ARRS roles.

Our Paramedics will work as a team to shape and deliver the future care for our patients who receive their care at home, working in collaboration with urgent and integrated community services who are leaders in admission avoidance and optimising patients’ lives within the community. The collective aim is to maximise our impact on the health and well-being of our local population.

You will have the opportunity to be involved in service improvement, implementing initiatives and directives locally with the support and supervision of your clinical leads within the PCN.  
 
As a Paramedic you will be expected to contribute to the everyday operations of the PCN working in collaboration with the PCN team to delivery high quality patient care. 

You must have access to your own transport, as this role will require travelling both to visit patients in their homes but also to a number of PCN practices and colleagues across Hounslow.

  • Registered Paramedic with 2 or more year experience in qualified practice
  • Completed or willing to complete FCP roadmap within the first year in post
  • Understanding of primary care and the ability to transfer current skills into a primary care home visiting role
  • Skill and experience assessing and managing clinical and non clinical risk with an understanding of relevant governance processes
  • Professional maturity and ability to self appraise, act on critical feedback and maintain high standards of practice
