Family Support Practitioner

Tŷ Hafan is a beautiful children's hospice located on the picturesque Vale of Glamorgan coastline. Covering the South, East and West Wales Tŷ Hafan offers a beautiful, newly refurbished working environment set in idyllic grounds a short distance from the metropolitan area of Cardiff.

We have an exciting opportunity for a Family Support Practitioner to join our well-established Family Support Team within our Family Wellbeing & Outreach services.

Hours for the role are 37.5 and are primarily Monday to Friday, 9am to 5pm but requires flexibility to cover regular on call duties outside of these hours. The primary base for the role is at the Hospice in Sully, but this role will focus on supporting families in the Cwm Taff Morgannwg area and so flexibility to travel across whole catchment area of South, East and West Wales is essential.

 

About the role

The role holder will be part of the wider Tŷ  Hafan family wellbeing service and will be expected to work in close collaboration with colleagues from family support, play & therapy and clinical colleagues based at the hospice and other areas across Wales. You will work closely with statutory colleagues in health, education and social care to ensure the holistic palliative care needs of the family are met. 

The role is responsible for assessing the need, and the delivery of emotional, social and practical support to families and children with life shortening conditions at the hospice, home and wider community, including hospital settings. This includes facilitating the delivery of a comprehensive pre and post bereavement service for each identified child and their family including the provision of psychosocial support, anticipatory grief support, end of life planning, memory making and advocacy.

Our team at Tŷ Hafan are supportive and inclusive, we value individual differences, and understand experience can come from many avenues, therefore it is not essential to have previous experience working in a palliative care environment. Palliative care is defined as the total care of individuals from diagnosis of a life-shortening condition, beyond transition into adult services, or in providing extensive bereavement support. It is important to remember that paediatric palliative care is so much more than just end of life care, Tŷ Hafan holds enhancing quality of life at its very core.
