Cardiac Specialist Nurse

• Primary and secondary mental health care for children and adults including specialist inpatient child and adolescent mental health provision, perinatal mental health, forensic services including low and medium secure care.
• Specialist community support for children and adults with learning disabilities and autism, including intensive support.
• Community physical health and well-being services for children and adults.

Clinic Based Services are looking to employ a motivated and enthusiastic senior Cardiac Specialist Nurse to join the Community Heart Failure team.
The post holder will work across the Trust as part of a team to support the equitable delivery of the Heart Failure Nursing Service, delivering an effective, efficient service for Heart Failure patients and their carers.

The post holder will:
- Work as an autonomous advanced practitioner to provide a high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients and carers
- Develop and promote a holistic, supportive and patient centred service that facilitates education, acceptance and lifestyle changes, which promotes self-management and aids recovery/improvement of heart failure
- Act as a resource person for patients, carers and other healthcare professionals and participate in the delivery of specialist education and training across the service area
- To act as a nursing lead expert for heart failure
- Promote partnership working to improve outcomes for patients and facilitate seamless transitions in care through primary, secondary and tertiary services
- Lead service development in community heart failure rehabilitation

The successful candidate will have evidence of cardiac nursing clinical experience equivalent to Masters level in a range of clinical settings including the community/ primary care setting, multi-disciplinary team /multi-agency collaborative working, proven experience of working with complex cases, and experience of working autonomously and managing own caseload.

- Degree in appropriate subject or evidence of education to degree level
- Current NMC registration

Experience

- Evidence of cardiac nursing clinical experience equivalent to Masters level in a range of clinical settings including the community/primary care setting
- Proven experience of working with complex cases
- Experience of working autonomously and managing own caseload
Skills and Abilities

- High level of specialist clinical knowledge including current evidence-based practice
- Highly competent in cardiac examination and to assess patient’s requirements to develop case management plans
- Knowledge of Expert Patients programme
