Clinic Based Services are looking to employ a motivated and enthusiastic senior Cardiac Specialist Nurse to join the Community Heart Failure team.

 The post holder will work across the Trust as part of a team to support the equitable delivery of the Heart Failure Nursing Service, delivering an effective, efficient service for Heart Failure patients and their carers.

Prif ddyletswyddau'r swydd

The post holder will:

  • Work as an autonomous advanced practitioner to provide a high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients and carers
  • Develop and promote a holistic, supportive and patient centred service that facilitates education, acceptance and lifestyle changes, which promotes self-management and aids recovery/improvement of heart failure
  • Act as a resource person for patients, carers and other healthcare professionals and participate in the delivery of specialist education and training across the service area
  • To act as a nursing lead expert for heart failure
  • Promote partnership working to improve outcomes for patients and facilitate seamless transitions in care through primary, secondary and tertiary services
  • Lead service development in community heart failure rehabilitation

The successful candidate will have evidence of cardiac nursing clinical experience equivalent to Masters level in a range of clinical settings including the community/ primary care setting, multi-disciplinary team /multi-agency collaborative working, proven experience of working with complex cases, and experience of working autonomously and managing own caseload.

  • Current NMC registration

Knowledge

  • Knowledge of current key NHS developments and policies and implications for service delivery

Experience

  • Evidence of cardiac nursing clinical experience equivalent to Masters level in a range of clinical settings including the community/primary care setting
  • Proven experience of working with complex cases
  • Experience of working autonomously and managing own caseload

Skills and Abilities

  • High level of specialist clinical knowledge including current evidence-based practice
  • Highly competent in cardiac examination and to assess patient’s requirements to develop case management plans
  • Knowledge of Expert Patients programme
