Hospital at Home Discharge Coordinator

West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Are you looking for an exciting change and for a secondment to a new service? Hounslow Community Healthcare has an exciting new role to support the ongoing development of the Hospital at home service. We are looking for an enthusiastic and motivated candidate with experience of managing complex discharge pathways and experience of working with multiple providers to provide co-ordination and leadership in line with agreed service objectives. 

The main aim of the service is to manage complex clinical cases at home, prevent unnecessary hospital admission and support early discharge of patients following an acute admission. We have short term funding until March 2026 to continue to run a Hospital at Home pathway. The postholder will work jointly with the Discharge Hub, Integrated Community Response Service as well as the London Borough of Hounslow to ensure patients are effectively supported through the Hospital at Home pathway.

Prif ddyletswyddau'r swydd

The Integrated Discharge Hub based within West Middlesex Hospital facilitates and tracks discharges across the D2A pathways and supports flow across the North West London (NWL) system.  The service is led by West London NHS and includes multiple providers; Chelsea and Westminster Foundation Trust, NWL ICB and London Borough of Hounslow.  It builds on the set up as part of managing COVID-19 identified in government guidance issued in March & August 2020 and subsequent guidance published in January 2024.

The team provides immediate assessment, intervention and management to a broad range of patients with complex needs. All members of the team work very closely together and have a good relationship with other community teams and clinicians such as district nurses, GPs, London Ambulance Service and mental health services.
