Band 4 Community Nursery Nurse for Vulnerable Families
Band 4
West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.
We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

The CNN is flexible to work across the 3 Hounslow localities depending on service need. The Community Nursery Nurse will offer health reviews and packages of care to families to families in temporary accommodation as directed by the specialist Health Visitors.
The work is offered in a variety of community settings, which include home, clinic, hotels, refuges, early years settings, Family Hubs & children’s centres. The CNN is flexible to work across the 3 Hounslow localities depending on service need
Prif ddyletswyddau'r swydd
To agree a planned programme of care in partnership with the parent /carer and health Visitor and implement this virtually or in the home or clinic setting.
To work within the skill mix Health Visiting team on own initiative, prioritising workload and seeking support and advice with named Health Visitor when necessary.
To update client information on Systm1.To input client information onto system1 according to guidelines, following clinical activity.
To communicate with the Health Visiting Team to ensure all health promotion public display notice boards/ websites are renewed and up to date.
Organise and support planned interventions virtually and in community settings, e.g. Well Baby clinics, breast feeding support groups, Children’s centres and other support groups.
To record in client progress notes appropriately as required and directed by the Health Visitor.
To work with the Health Visiting team in the completion of 1 and 2 year
developmental reviews in the home/ hotel/ Family Hub & children’s centre /clinic and establish a system for inviting these children for the review. To book children into sessions and record non-responses.
To carry out child development reviews as per the healthy child programme using the Ages and Stages Questionnaire in group settings and individually in accordance with the Healthy Child Guidelines .
To liaise with HVs, GPs and Early Years settings following health reviews.

Qualifications

- NNEB /Diploma in Child Care (DCE) awarded by CACHE & NHC in Child Care in Scotland. NVQ level 3. Other relevant qualifications may be considered.

- Additional training in child development.
- Safeguarding training.
Experience

- Two years’ experience post qualification.
- Experience of working with parents/carers.
- Experience of working as part of a 0-19 Team.
- Community experience.
