Continuity Team Leader
We are seeking an enthusiastic midwife for an act-up opportunity to cover maternity leave. You will lead one of our continuity teams, in Farnham, from October 2025 for a year.
This position is open to midwives who are interested in working within a continuity model of care and are able to meet the challenge of leading a team to deliver high standards of care, ensuring that each woman’s journey to motherhood is an individualised experience.
The role will encompass both clinical shifts in and out of hospital, an administrative element and will require an in hospital on call commitment.
You must hold NMC midwifery registration.
You will be expected to be able to show evidence of current practice/updating.
Main duties of the job
• The role will be part clinical to support the team rota and provide clinical supervisory support to midwives to realise their full potential as autonomous practitioners placing women, babies and their families at the centre of care on a continuity of care pathway.
• The post holder will work closely with the Senior Team in maternity services and work collaboratively with specialist midwives, the obstetric team, health visitors and GP practices to further develop pathways so that services are responsive to the needs of women and families.
• The women and their families' experience of our services will be dependent on high quality, effective and customer focused care and the post holder will ensure that care provided is in keeping with the Trust’s vision and values.
• Maintenance of a twenty-four hour maternity service for women and participate in on-call commitments as required. The post-holder will be required to work in both the community and the hospital across a 7 day service including night duties.
• The post-holder is expected to supervise junior staff and to be able to teach qualified and unqualified staff, including basic and/or post-basic students.
The Continuity Team Leader will provide clinical leadership, facilitate professional development and operationally manage one or more continuity midwifery teams.

Detailed job description and main responsibilities
The Royal Surrey NHS Foundation Trust is looking for an experienced, enthusiastic midwife to join our innovative and award winning maternity service.
An exciting opportunity has arisen for a band 7 position to provide leadership to ensure effective management of continuity of carer/case loading teams. The teams operate in a shift based system and provide care across the full maternity pathway. This provides enormous job satisfaction, excellent clinical outcomes and a positive and personalised patient experience. The model of care enables midwives to build relationships with the women and families in their care, seeing them at home, in the bespoke maternity hubs and on the maternity unit.
This post holder will need to be highly organised with an ability to maintain safe provision of care for women within the team. This will include reviewing clinic capacity, activity and staffing and supporting the clinical provision of care for a proportion of the contracted hours. The post holder will be motivated and demonstrate an ability to facilitate exceptional patient experience as well as provide robust leadership to the midwives within the team.
