Patient Pathway Coordinator Lead, Specialised Cardiology

Trosolwg o'r swydd

The second part of the role focuses on the outreach programme ELOPE which leads and supports management of CVD risk factors and health promotion in the community for both primary and secondary prevention. This involves working with schools, employers and media

This role will be split into two main areas of focus – one supporting existing clinical services for  cardiovascular disease (CVD) prevention, and one supporting the CVD prevention outreach programme of East London Cardiovascular Disease Prevention (ELOPE).

The existing clinical prevention services include CVD risk & lipids, clinical pharmacology and hypertension, and cardio rehab. as well as closely with the Integrated Care Board. There is also an expectation to support with funding applications, for research grants or other bids

to provide high quality care to patients. East London Cardiovascular Disease Prevention within the Specialised Cardiology directorate, alongside seven other specialist services.

Experience

  • Experience using Microsoft office packages to produce report, spreadsheets and presentations
  • Experience in service improvement
  • Healthcare experience
  • Experience with, and a creative approach to, problem solving and responding flexibly to planning and change.
  • Experience or demonstrated involvement with health promotion activities

Knowledge and Skills

  • High-level interpersonal skills including motivational negotiating, influencing and relationship building and ability to work creatively with diverse teams.
