An excellent opportunity has arisen for an enthusiastic, senior clinical pharmacist to join the Neurosciences Team at Charing Cross Hospital. The post-holder will provide clinical care for patients on our Hyper Acute Stroke Unit and neurology wards, as well as support outpatient prescribing and supply of medications within our neurology services.

The post-holder will rotate between the Stroke Service and Neurology Service on the Charing Cross Hospital site. The successful candidate will provide a ward-focused service to a specialist cohort of patients. You must have excellent communication skills, be self-motivated and enjoy a challenge.

You will identify patients’ pharmaceutical needs, assemble medication histories, reconcile medication charts, and facilitate ward-based discharges incorporating medication counselling on medication changes.

You will have the opportunity to lead on service improvement and audit projects, as well as the development of clinical guidelines and standard operating procedures.  You will become a key member of the ward multidisciplinary team, supporting the work of both the wards and directorate.

Ideally, you will have completed your clinical diploma and hold an independent prescribing qualification, however we can offer appropriate training if needed. The successful candidate will additionally participate in general departmental duties i.e. late duties and weekend/bank holiday working in the inpatient dispensary, as per rota requirements.

  • To deliver and assist in the provision of a high quality, patient-focused clinical pharmacy service to both inpatients and outpatients, in line with local and national guidelines.
  • To assist the lead pharmacists in the management and development of medicines management initiatives.
  • To assist the lead pharmacists in the management and development of trainee pharmacists when on rotation within stroke and neurosciences.
  • To provide clinical pharmacy services, both department and ward-based, in line with trust procedures and national legislation.
  • Facilitate the efficient and cost-effective use of medicines 
    Support other healthcare professionals with the prescribing and management of medicines.
  • To undertake accompanied ward visits where appropriate, and provide support to, junior pharmacists as directed by the lead pharmacists

Education

  • Master’s degree in pharmacy (MPharm) or equivalent
  • Active registration as a pharmacist with the GPhC
  • Completed, or currently undertaking, a postgraduate diploma in general pharmacy practice, and have satisfactorily passed all assessments pertaining to the postgraduate certificate modules (i.e. 12-18-month assessments) and beyond
Meini prawf dymunol
  • Full Postgraduate Diploma in General Pharmacy Practice (completion of all FS2 modules of the JPB/UCL diploma) or another equivalent postgraduate diploma
  • Independent prescriber annotation on the GPhC pharmacist register

Experience

  • Solid grounding in all aspects of hospital pharmacy practice (medicines information, aseptic services, ward services and dispensing)
  • Experience in delivering education and training
  • Experience in the management of others
  • Demonstrate the ability to appropriately recommend, substantiate and communicate therapeutic options for patients

  • Experience of clinical guideline development
  • Experience of planning and developing pharmacy services
  • Previous experience of using electronic prescribing systems
  • Previous experience of managing a specialist pharmacy service
