West London NHS Trust  provides a full range of mental health, community  and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.

We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Do you want to be part of a team making Hammersmith & Fulham a great place to grow old?  The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.  We are looking to recruit an enthusiastic and self-motivated clinician, who will be an integral member of the team.

 You will have excellent communication skills, experience completing person-centred holistic physical and cognitive assessments and care for people with moderate to severe frailty.  You will have good organisational skills and be able to manage your own workload to meet competing priorities. The role involves collaboration with internal and external stakeholders across the system. You will work as part of a team and delegate work to the team coordinators and be supported by local geriatric consultants and General Practitioners.  You will also be a part of our newly developed neighbourhood team in alignment with the NHS 10 year plan.

Working as a member of the Community Independence Service you will be skilled in completing physical observations such as News 2 and relevant holistic assessments and mental capacity assessments, identifying whether the person has deteriorated and taking appropriate action.  You will organise and chair Best Interest meetings for those who require support expressing their wishes.

 You will attend local Primary Care Network (PCN) meetings/Integrated care access point meetings (ICAP) to contribute to the health and social care needs of local residents, identifying cases suitable to support through the CIS Case Management caseloads. You will also provide clinical advice and support CIS internal teams such as Urgent Community Response, Home first and rehabilitation.

 You will provide clinical and operational supervision to the health and social care coordinators in case management and deputise for the pathway leads at meetings or in their absence alongside your CIS colleagues

Experience

  • Previous clinical experience in the following: • Older people / frailty assessment and rehabilitation • Medicine and general surgery • Orthopaedics • Falls • Dementia • Long term conditions
  • Substantial experience of working at a band 5 level (acute / community) as an occupational therapist or physiotherapist
  • Experience teaching junior staff members /rehab or therapy assistants / students.
  • Experience completing audit and service development
  • Experience of providing clinical supervision

  • Experience of advance care planning and End of Life care identifying and managing clinical situations working with universal care plan (or similar system)
  • Previous general experience to include the following: • Community working • Neurology • Pain management • Frailty or older adults

Knowledge and Skills

  • Excellent communication skills, fluency in written and verbal English
  • Ability to maintain accurate, timely, concise and legible clinical records both electronic and handwritten which adhere to the Trust policies.
  • Ability to talk to people about sensitive issues in a supportive manner but also challenge others assertively where necessary.
  • Knowledge and ability to use Microsoft Office suite for various administration tasks, including spreadsheets, document writing and email.
  • Knowledge and ability to use patient record systems such as SystmOne or similar.
  • Understanding and ability to apply relevant knowledge and skills required to act as an effective professional clinician and adhere to your relevant code of conduct/practice.
  • Ability to plan and use assessment methods that are appropriate for complex needs, and uses processes of reasoning that are based on available evidence including national guidelines such as NICE and other local and professional policies.
  • Ability to follow processes of reasoning which balance additional information against the overall picture of the individual’s needs to confirm or deny developing hypotheses
