We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Hounslow Community Psychosis Rehab Team: Band 6 Care Coordinator

Thank you for your interest in applying for this B6 Care Coordinator position within our Hounslow Community Rehabilitation Service (CRS).  This is a full time and permanent position and attracts 15% inner London weighting.

West London NHS Trust’s Community Rehabilitation Services has been operational for approx. 3 years and has recently gone through a period of review. We are the process of implementing the findings of the review and so there will be a shift from the Care Coordinator role towards named worker which will allow for more specific professional clinical input to occur. The emphasis of the changes is to ensure that the service meets and exceeds the current quality standards for community rehab for psychosis and aims to provide the highest standards of clinical care possible. Given these changes, we would strongly recommend that you contact us for a pre-application conversation, so that you are adequately prepared for the upcoming recruitment process.

What we are looking for are highly motivated and enthusiastic clinicians with a passion for psychosocial rehabilitation who are excited by service development. The successful applicant will be supported by senior and professional management, as well as working within a highly engaged team who are keen to continue to make positive changes. Previous experience of rehabilitation is less important than a willingness to learn and commitment, drive and creativity in meeting the challenges for the team as we move into the future.

Qualifications and Training

  • Current professional qualification in Mental Health Nursing/Social Worker/OT.
  • Registration must be live with a professional body such as NMC or other relevant body
  • Evidence of post registration training and CPD

  • Family Therapy/Work training.
  • Other relevant training, e.g. substance misuse

Experience

  • Experience of working in multi-disciplinary mental health teams with clients with severe and enduring mental health problems
  • Knowledge of CPA process
  • Excellent verbal and written communication skills, including report writing and presentations / IT skills.
  • Ability to assess, plan, implement and evaluate a comprehensive care package for the identified client group, using a wide range of resources.
  • Able to develop a comprehensive programme of intervention and support for the identified client group and their families
  • Skills in risk assessment and management.
  • Can demonstrate understanding of psychosis and its impact on service users and carers.
  • Demonstrate knowledge of current legislation and guidance applicable to this client group
  • Can demonstrate understanding of the needs of diverse groups, e.g. residents of inner city; black and minority ethnic groups, and skills in engaging with them.
  • Skills in psychosocial interventions
