Trosolwg o'r swydd

We are seeking an experienced and highly motivated Partnership director. It is an exciting opportunity to make a real difference. This is a key system leadership role working across the borough with health and social care, voluntary and community sector partners to help create a more joined -up effective and sustainable health and social care system for residents in Hounslow. Hounslow is a vibrant and diverse borough in West London, facing both opportunities and challenges in meeting the health and social care needs of its population. The Hounslow Borough Based Partnership (BBP) is a group of local NHS partners and local authority coming together to work collaboratively to make it easier for Hounslow residents to access and navigate health and care services within the borough of Hounslow. As one of eight BBPs within the Nort West London Integrated Care System (ICS), the Hounslow BBP focuses on improved health efforts for a population of approximately 300 residents. It is estimated that a total of approximately £393 million is spent annually on health and care for residents in Hounslow. The BBP brings together health, social care, and wider Local Authority services to remove barriers to healthcare for our communities, creating the right conditions for local partnerships to thrive.
