Advance Your Compassionate Career: End of Life Care Nurse
Do you possess exceptional communication skills & are driven to ensure every patient experiences dignified, high-quality End of Life Care?
We're seeking a highly motivated & experienced clinician to join our End of Life Care Team in Merton as a Community End of Life Care Nurse. This is a unique opportunity to shape quality palliative care across our community.
Why this role is unique and impactful:
- Pivotal Role in Quality: This isn't just clinical delivery, it's about facilitation, mentorship, and policy implementation. You'll be an essential link, working collaboratively to embed best practice & Coordinated Care across primary care, social services & care homes.
- Expert Autonomy: While supported by the Band 7 Specialist Nurse, you'll work with a moderate degree of autonomy, utilising your clinical judgement & discretion to provide direct advice & mentorship to front-line staff caring for dying patients.
- Strategic & Clinical Development: This post offers an immediate chance to develop your clinical leadership & strategic skills. You'll be instrumental in managing a patient caseload to facilitate best practice & collecting data to support key performance indicators (KPIs) for End of Life Care

- Registered Nurse with full and current UK NMC registration
- Evidence of continuing personal development and training

- Related Diploma/Module in EoLC, Palliative Care, Care of the Dying
Experience

- Post-registration experience
- Previous experience of palliative care / Previous experience of the End of Life Care tools (Gold Standards Framework and Advance Care Planning, Coordinate My Care)
- Experience of working in the community
- Familiar with EoLC medications and how to administer via subcutaneous route
- Demonstrate a knowledge of national policies influencing end of life care
- Experience, insight and understanding of Clinical Governance, risk management, clinical effectiveness, audit, patient and public involvement initiatives
- Competent using the Universal Care Plan

- Teaching experience in formal and informal settings
- Recent experience of involvement in management of patients moving between acute care and community
