The Trust was first established in 2002 and employs approximately 7,500 staff who provide care from more than 400 sites. Don't miss out on this exciting opportunity to join the expanding joint working across Home First and Urgent Community Response Services to facilitate complex hospital discharges and to support and keep patients safe in their own home.

We are a team of Occupational Therapists Physiotherapists and advanced clinical practitioners working closely with the multidisciplinary team to prevent further deterioration in patients’ physical functioning and wellbeing. We cover the areas of Chorley, Preston and South Ribble.

Your role will be to support the team including undertaking non-complex assessments and carrying out treatment plans under the direction of the therapists.

We are looking for a flexible Technical Instructor/ Assistant Practitioner who is able to work across a range of settings and has experience working to support Occupational Therapy and Physiotherapy interventions within Home First and Urgent Community Response Teams.

The service will operate 365 days a year, 8.00am - 8.00pm. 

We are looking for a Technical Instructor/ Assistant Practitioner with excellent communication skills and who has the ability to work autonomously as well as part of a team.

You will have regular supervision and professional development opportunities.
