West London NHS Trust provides a full range of mental health, community and physical healthcare services for children, adults and older people living in the London boroughs of Ealing, Hammersmith & Fulham and Hounslow.
We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

Hammersmith & Fulham a great place to grow old. The Community Independence Service (CIS) has an opportunity for a Band 6 Physiotherapist to join our rehabilitation team.
The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.
We are achieving excellent results in community rehabilitation and admission avoidance for the people of Hammersmith and Fulham and are part of a wider community independence service offered in Westminster and the Royal Borough of Kensington & Chelsea

As a Band 6 Physiotherapist, you will provide an integrated Health & Social Care service for people at home who are at risk of being admitted to hospital unnecessarily, providing Community Rehabilitation, Rapid Response, Reablement and Supported discharge. You will be working closely with our acute partners, such as Imperial College Healthcare NHS Trust and Chelsea & Westminster Hospital NHS Foundation Trust.
We will connect you with peers across the tri borough of Hammersmith and Fulham, Westminster and Kensington and Chelsea to support you and to work on service improvement partnerships.
We will ensure you are able to follow the clinical and leadership development priorities identified in your personal development and appraisal plan.
We will equip you for mobile working and actively support flexible working practice when required and appropriate for the service.

Experience

- Previous experience of working in the UK or a similar health economy (acute or community) as a physiotherapist.
- Previous general experience to include the following: · Older people/frailty assessment and rehabilitation · Medicine and general surgery · Orthopaedics · Falls · Dementia · Long-term conditions
- Experience teaching juniors and assistants
