Are you a dynamic, forward thinking Occupational Therapist who embraces interdisciplinary working and is passionate about Neuro Rehabilitation? A part time (22.5 hours per week),  permanent Band 6 Occupational Therapist role has becomeavailable within the Community Based Neuro Rehab Team and Earlier Supported Discharge Team for Stroke working across the East of Berkshire.

You will work alongside a supportive and forward thinking team within the East locality comprising a Band 7 Occupational Therapist, Neuro Psychologists, Physiotherapists, Speech & Language Therapists & Stroke Nurses. The team delivers an interdisciplinary approach to Stroke Rehabilitation working closely with our acute partners, social services and the voluntary sector.

The East Early Supported Discharge Team forms part of the wider Community Based Neuro Rehab where you will be part of a network of Neuro Specialist OTs who have an established in-service training programme and work together on innovative service development projects such as Visual Rehab and Driving Pathways. There is support for personal growth and development of clinical skills within Neuro Rehabilitation.

You will visit patients in their own homes so you will require a driving license and daily access to a vehicle to travel between patients. You will complete solo visits as a lone worker as well as joint visits with Therapy Assistants and other professionals.

Prif ddyletswyddau'r swydd

  • Use specialist Occupational Therapy skills for patients with Neurological Conditions and work with other members of the MDT to maximise rehab outcomes for patients and support them to achieve meaningful goals

  • Responsible for assessment, differential diagnosis, formulation of treatment plans and writing of full and clear clinical records and assessment reports, reflecting clinical reasoning

  • Work with other agencies and services to support patients in the management of their condition including their care, social, postural and equipment needs

  • Work together with members of the team across the MDT to support clinical decision making around patient care and triage of new referrals

  • Contribute service development projects, evaluation and audit

  • Supervision, training and support of other Therapy Assistants and students

  • Work with other OTs and the MDT on service development projects, support training and clinical excellence within the team with the support of senior colleagues

  • Responsibility for maintaining own competency and effectiveness of practise through relevant CPD, knowledge of clinical guidelines and research

  • Maintain a high level of professional responsibility by following relevant trust and professional standards and policies
