We are seeking a motivated and enthusiastic pharmacist to join our progressive and forward-thinking pharmacy team. In this role, you will provide high-quality, specialist pharmaceutical care to inpatient and community mental health service users across Leeds. This will include delivering complex clinical interventions, providing expert medicines information, and contributing to education, training, and service development.

The service spans four inpatient sites, each with a defined subspecialty, alongside three community-based hubs supporting the wider Leeds population.

You will work as an integral member of multidisciplinary clinical teams, holding a high level of autonomy to support medicines optimisation. You will be expected to work both independently and collaboratively within ward and community settings, making autonomous clinical decisions within your scope of practice, with access to senior support when required.

As a Band 7 Specialist Clinical Pharmacist, you will use your clinical knowledge and skills to advise healthcare professionals on the safe, evidence-based use of mental health medicines.

 

Prif ddyletswyddau'r swydd

•    To deliver, maintain and further develop a high-quality clinical pharmacy service to Community Mental Health Teams (CMHTs) across Leeds, supporting the Lead Pharmacist and Advanced Clinical Pharmacist for community services.
•    To act as a clinical specialist in mental health pharmacy, providing expert advice to the multidisciplinary team (MDT) on the safe, effective, and evidence-based use of medicines, including practising as an Independent Prescriber where appropriate.
•    To take responsibility for a defined clinical area, as part of the wider pharmacy service.
•    To provide a comprehensive, patient-centred clinical pharmacy and medicines optimisation service to service users under the care of Leeds and York Partnership NHS Foundation Trust (LYPFT), across both community and inpatient settings.
•    To work closely with primary care and wider healthcare teams, providing medicines information, advice, and guidance to both healthcare professionals and patients to support continuity of care.
•    To support safe and effective discharge planning, liaising with inpatient and primary care colleagues to ensure safe transfer of care and adherence to shared care guidelines.
•    To deliver high-quality, patient-focused pharmaceutical care, ensuring safe, effective, and evidence-based use of medicines and improved clinical outcomes.

The Leeds and York Partnership NHS Foundation Trust (LYPFT) is the main provider of Mental Health and Learning Disability services in Leeds. We also provide specialist inpatient services in York and some highly specialised services across the country. As a teaching trust with strong links to local universities, we are a centre of excellence for teaching, research and development.

•    To provide timely and accurate medicines information and specialist advice to healthcare professionals, patients, carers, and families, promoting understanding, adherence, and shared decision-making.
•    To actively contribute to multidisciplinary teams, including attendance at ward rounds, clinics, and case discussions, where pharmacy input is valued and integral to patient care.

•    To support the education, training, and development of pharmacy professionals, trainees, and wider MDT staff across the Trust, contributing to structured teaching and informal learning.
•    To contribute to the physical health and wellbeing agenda for people with mental illness, supporting Trust and national priorities to reduce health inequalities.

•    To participate in clinical audit, quality improvement initiatives, and service development activities to enhance patient care and pharmacy services.
•    To contribute to pharmacy team meetings, governance processes, and the ongoing development of clinical pharmacy services.
•    To support service innovation and improvement, working collaboratively across teams to identify and implement changes to practice.
•    To participate in the pharmacy rota, including Saturdays, Bank Holidays, and on-call duties as required.

You will also contribute to the supervision and training of staff, support service improvement initiatives, and take an active role in governance, quality improvement, and audit.

This role offers excellent opportunities to further develop your clinical expertise in mental health, alongside leadership, education, and operational management skills. You will be encouraged to contribute to service innovation and workforce development.

Previous experience within secondary care is desirable but not essential. Applicants must demonstrate strong organisational, communication, and problem-solving skills, as well as a clear interest in mental health. Applications are welcomed from pharmacists across all sectors.

Leeds and York Partnership NHS Foundation Trust provides a wide range of general and specialist mental health services.
