Are you an experienced Community Nurse wanting a new and exciting Challenge? This post is to work within the Community Nursing team to deliver unscheduled care to avoid unnecessary hospital admissions and ensure patients receive a two hour response when clinically indicated.

Working within the Community Nursing team as a senior nurse you will support the Community Nursing team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers, ensuring unnecessary hospital admissions are avoided and ensuring patients known to the Community Nursing team receive a two hour response when clinically indicated.
Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery Council. The successful applicant will work within the Community Nursing service to avoid unnecessary hospital admissions and ensure patients receive a two hour response when clinically indicated.
Job Purpose:
The post holder will ensure the provision of the highest standard of clinical care by using advanced skills and expert knowledge to holistically assess needs, instigate, and provide clinical treatments using evidence-based practice to prevent hospital admissions.
The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and at the end of life for physical and symptomatic nursing care
The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working
To ensure the Urgent Community Response Two Hour National standard is met for patients on the Community Nursing caseload with an urgent need.

Qualifications

- Registered Level 1 Nurse with current Nursing and Midwifery Council registration
- Practice Supervisor and Assessor (to be completed within 1yr of post)
Skills, Knowledge & Experience

- Band 6 experience in the community setting
- Ability to prioritise visits in accordance with patient need and to organise workload without direct supervision
- Experience prioritising, and allocating assessments in accordance with patient clinical need and individual competencies
- Broad range of clinical skills relevant to Community Nursing, - including but not limited to Compression Bandaging, Dopplers, Venepuncture, Male & Suprapubic catheter, Syringe drivers and Palliative care Symptom Management.
- A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010
