Main duties of the job
A fantastic opportunity has arisen to be part of our well-established Community Nursing team in Bracknell, as a Community Matron.
The post holder will work full time within an integrated community nursing team to proactively manage a complex caseload of high-risk patients, applying advanced clinical skills, clinical reasoning and practising autonomously.
The post holder will promote shared decision making with individuals regarding their care plan and encourage proactive self-management of long-term conditions with the aim of avoiding crisis resulting in unplanned hospital admission.
The post holder will work be fully integrated into the locality community nursing team, working with other stake holders to manage complex patients, as well as the wider multi-disciplinary team in a variety of settings to improve health outcomes and quality of life for patients with multiple chronic diseases.
The role requires the applicant to take a lead role in managing patients with complex needs to avoid hospital admission and access services as required. The post holder will be a senior nurse supporting community nursing using established independent prescribing and advanced assessment skills, upskilling junior members of staff and supporting scheduled caseloads.

The “must haves” for this role:
- Able to use advanced assessment skills and expert knowledge to perform comprehensive assessments of physical, mental health and social care needs that addresses the combined impact of all of the patient's conditions in order to improve health outcomes and quality of life. To exercise leadership and judgment to ensure good clinical practices and maintain high standards of care and review these at regular intervals.
- Qualifications in Independent Prescribing and Advanced History Taking and Physical assessment.
- Able to identify and plan preventative measures and anticipatory care needs.
- Have a high level of clinical knowledge and clinical reasoning skills.

Education, qualifications or training
Essential criteria
- Registered General Nurse
- Post registration Advanced Clinical Study e.g. diagnostic reasoning
- Non-medical Prescriber Level 6/7
- Current NMC Registration
- Evidence of continuing professional development
- Experience of teaching, mentoring, supervising
- Qualification in teaching and assessing
Desirable criteria
- To have undertaken study at master’s level or willing to do so
Previous Experience
Essential criteria
- Substantial post registration
- Experience of working in primary or community health setting
- Experience of complex case management
- Extensive knowledge of older people’s and long-term conditions agenda
- Demonstrate a high level of clinical skills
- Demonstrate an in-depth knowledge and experience of clinical examination, history taking, pharmacology, pathophysiology, etc.
- Experience of instigating and interpreting investigations and initiating appropriate plans of care for patients
- Experience of promoting self-care
- Cultural awareness
- Experience of multi-agency working across service interfaces
