Are you interested in caring for people who have experienced falls? If so, come and work with us in the Specialist Falls Service, we are a friendly and busy team who provide care for those at risk of falls across Bristol, North Somerset and South Gloucestershire.

What do we do?

We deliver community specialist falls services, including specialist advice and guidance to people in our community, Integrated Network Teams, Acute Trusts and Primary care/ HCPs within Sirona and Community.

We provide advice and guidance; self-management strategies through face to face clinics and other media means to all professionals, patients and members of the public.

Current working hours of the service: 8 - 5 pm 5 days a week  Flexible working base for clinical staff but you will be expected to travel to patient’s homes, clinics, community settings and cover BNSSG wide.

Main duties of the job

  • Undertake a range of delegated clinical, care and therapy led activities to support the service users on the Specialist Falls Team caseload.
  • Undertake assessments and treatment plans for service users as delegated by senior members of the team
  • To provide a safe, patient centred, effective and evidence-based care. To deliver a full range of clinical, health and social care activities, supported by a robust competency framework to service users within their own home and other settings
  • You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, physiotherapists, occupational therapists and administrators.
  • Work flexibly to meet the service needs and undertake any other duties that may be reasonably requested to help the service run smoothly.
  • To work shifts over 5 day period between the hours of 08.00 –17:00 in order to meet the needs of the service.
  • Travel throughout the service delivery geographical area
  • Work as part of the Specialist Service delivering clinical and therapeutic care, within set protocols and within scope of Band 3 Practitioner remit.
  • To undertake and complete appropriate training and competencies in order to carry out the role of Band 3 Practitioner.
  • Escalate to registered staff for support and guidance when required.
