Job overview

Are you an enthusiastic and highly motivated Nurse who is ready for a challenge, the West Stoma / Colorectal Nursing team is seeking a highly self-motivated practitioner, with a desire to promote and develop clinical excellence in clinical practice. The successful post holder will join an established Stoma / Colorectal Nursing team and provide nursing support to inpatient / outpatient areas.

 

Main duties of the job

The Triage Nurse will play a key role in managing colorectal referrals within secondary care, supporting the delivery of the Straight to Test (STT) pathway. The post holder will be responsible for assessing and triaging referrals, ensuring patients are appropriately managed and directed to the most suitable pathway. The role will focus on optimizing patient flow, reducing unnecessary delays, and enhancing the quality of care.

The post holder will work within the multidisciplinary team (MDT) to support the timely assessment and management of patients referred with suspected colorectal conditions, ensuring that national and local pathway targets are met.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Detailed job description and main responsibilities

You will need the following:

  • Registered Nurse with current Registration with the N.M.C.
  • Post Registration Education, a Diploma / Degree Level
  • Post-Registration nursing experience at Band 5 level or above
  • Demonstrate involvement in change management and successful implementation of practice / quality improvements.
  • Excellent Organisational and Leadership skills.
  • Experience of multi professional working / working effectively with other stakeholders
