Are you looking for a new challenge? We are looking for a highly motivated individual to join our team to support effective patient management for our Surgical and Medical wards within Ysbyty Gwynedd. This is an exciting opportunity for a dynamic individual who will be a key resource in supporting our ward teams.
Main duties of the job
To support effective patient management for our Surgical and Medical wards within Ysbyty Gwynedd.
The post involves working in a multi-disciplinary environment and will support systems and processes to positively impact patient management.
They will be an integral component of the Directorate Teams, working closely with the Ward Staff to facilitate a safe and effective timely discharge/transfer and review of ‘outlying’ patients from hospital.
The post holder will also need to work alongside the Clinical Site Managers to ensure that bed use is maximised.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
