Band 5 Registered Nurse – Ward 14 Discharge Unit

Ward 14 Discharge Unit is a dedicated area supporting patients who are medically fit to leave the main ward but require short-term care or coordination before going home or to another care setting. The unit ensures safe, timely discharge, improves patient flow, and helps reduce delays and hospital-acquired infections.
 
The ability to speak Welsh is desirable; however, English and/or Welsh speakers are equally welcome to apply.
 
 

Main duties of the job

As a Band 5 Registered Nurse on the Discharge Unit, you will provide high-quality, compassionate care to patients who are medically fit for discharge but require short-term support before leaving hospital. You will:

  • Assess and monitor patients to ensure safe, timely discharge.
  • Coordinate care plans, medications, and follow-up arrangements.
  • Work closely with multidisciplinary teams to improve patient flow.
  • Maintain infection prevention standards and best practice.
  • Support patients with complex needs

Detailed job description and main responsibilities

The Band 5 Registered Nurse will work within Ward 14 Discharge Unit, a dedicated area supporting patients who are medically fit for discharge but require short-term care or coordination before leaving hospital. This role is essential in improving patient flow, reducing delays, and ensuring safe, timely discharge while maintaining high standards of infection prevention and patient care.

Deliver high-quality, compassionate nursing care to patients awaiting discharge.Assess and monitor patients to confirm clinical readiness for discharge.Coordinate discharge plans, including medications, follow-up appointments, and community support. Work collaboratively with multidisciplinary teams to facilitate smooth patient transitions. Uphold infection prevention and control standards at all times. Support patients with complex needs during their hospital stay. Contribute to reducing hospital-acquired infections through best practice care.
