We are seeking an Advanced Clinical Practitioner to join our Urgent Community Response (UCR) team in South Bristol!

We want an experienced Nurse who is used to working creatively to assess, diagnose and implement treatment plans for people who require crisis intervention within a community setting. You will work alongside a other health care professionals to manage clinical risk for these patients in the community.

Your medical knowledge will help to reduce unnecessary hospital admissions and contribute to effective admission avoidance when service users are in acute need. The post holder will be expected to contribute to service improvement initiatives.

You will be part of the point of clinical escalation rota in the South Bristol locality, using virtual assessment skills to support our staff members who are face-to-face with deteriorating or acutely unwell patients.

You will be become a part of our Integrated Neighbourhood Teams working out of the Withywood Centre and Knowle Clinic and covering the whole of South Bristol.

Due to the qualification criteria for this role requiring a Non-Medical Independent & Supplementary Prescriber (or willingness/ability to work towards) , we are currently unable to accept applications from Occupational Therapists.

Main duties of the job

As an ACP, you will work autonomously as part of the Urgent Community Response Pathway (UCR) within our Integrated Neighbourhood Teams.

The role involves effective history taking, clinical assessment, diagnosis, and the implementation of a range of interventions for patients presenting with acute undifferentiated and undiagnosed conditions and acute management of chronic long-term illnesses. The post holder will be able to demonstrate effective management of clinical risk within the community to support patients to remain in their own home.

This will include prescribing and reviewing medication appropriate to the needs of patients, within your scope of practice and in accordance with national and local protocols. It will also include overseeing of patient caseloads, senior clinical leadership for more junior staff and wider input into clinical leadership within our locality, working closely with other B7 clinical colleagues and Operational Management to support the delivery of effective and safe clinical standards of practice.

The successful ACP will need to demonstrate critical thinking in the clinical decision-making process; identify gaps within the service; and contribute to ongoing service improvement and development in line with national and local strategies.

You will be required to work collaboratively with other members of the multidisciplinary team to establish a whole system approach to managing health care for individuals with complex care needs.

This post is not an Advanced Practice role that is mapped to the HEE Multi-professional Framework for Advancing Practice. These posts are currently under review and are likely to undergo a name change in the near future. This is because these roles do not require candidates to work across all 4 pillars of Advanced Practice to MSc level and are therefore banded at AFC Band 7. They do, however require a skillset working at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not be eligible for training funding for an MSc in Advanced Practice however training may be supported depending on individual learning needs identified following Sirona CPD funding application processes.

Skills, Knowledge & Experience

Essential criteria
  • Knowledge of local, national, strategic and operational policy developments and an awareness of the implications of the same.
  • Understanding of the primary care, secondary care and provider interface.
  • Ability to effectively communicate complex clinical information to a diverse patient group.
  • Ability to prioritise against tight deadlines and work under pressure.
  • Ability to effectively motivate others.

Qualifications

Essential criteria
  • Non-Medical Independent & Supplementary Prescriber or willingness to undertake Pharmacology and Prescribing Module at Masters Level.
  • Physical Examination and Diagnostic Reasoning Module (or equivalent) at Masters Level or willingness to undertake.
Desirable criteria
  • Have or working towards practice educator qualification or equivalent.
  • Teaching and Assessing qualification/ experience
  • Evidence of Master’s level clinical qualification
