We are seeking a Band 4  Assistant Practitioner Practitioner to join our Physiotherapy team in the  Adult Learning Disability Health (ALDHS) Service.

The service has 3 locality multi-disciplinary teams covering, Bristol, North Somerset, and South Gloucestershire. This position will include a caseload  across all localities.

The service prides itself on delivering high quality support to service users with a variety of physical and mental health issues alongside their Learning Disability. You will also have opportunity to work alongside our mainstream health colleagues to enable people with a learning disability improved access to health care, through training, reasonable adjustments, resources, and liaison.

Service hours: Monday to Friday, 9am to 5pm

 

Prif ddyletswyddau'r swydd

  • Undertake a range of delegated clinical, care and therapy led activities to support the service users on the adult learning disability health service caseload. This will include hydrotherapy.
  • Undertake assessments and treatment plans for service users with physiotherapy needs as delegated by senior members of the team
  • To provide a safe, patient centred, effective and evidence-based care
  • To deliver a full range of clinical, health and social care activities, supported by a robust competency framework to service users within their own home and other settings
  • You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, speech and language therapists, occupational therapists and physiotherapists
  • Assist in the provision of training sessions and creating related resources.

  • Successful completion of relevant Foundation Degree modules / Diploma in Clinical Health and Social Care / diploma level 3 or equivalent experience.

Experience

  • Able to use a variety of IT platforms i.e. EMIS; (emails, word documents, excel, power point and publisher)
  • Experience of direct client care in a health or social care setting.
  • Experience of working with people with a learning disability who may have mental health needs, physical health needs, and display behaviour that challenge

  • Experience of developing accessible resources
  • Able to work effectively with key agencies (day-care, college, tertiary services)
  • An understanding of the issues of equality and diversity
  • An understanding of team dynamics, including what factors make a team work well and what can go wrong and why.
