We are seeking practitioners to join our established Learning Disability and Autism liaison team across . The successful applicant will be passionate about ensuring equitable access to our acute services for people with learning disabilities and autistic people.
Excellent knowledge of the range of reasonable adjustments that can be provided and a commitment to advocate for people is essential for this role. The ability to tailor your communication to work effectively with service users, families and professionals including in multi-disciplinary meetings and when providing training is a key skill.
Prif ddyletswyddau'r swydd
You will be an expert practitioner to support acute hospital staff to meet the needs of people who have complex needs through innovative provision of reasonable adjustments.
You will be able to work autonomously and prioritise your workload according to capacity and demand.
You work collaboratively with the acute Trust on agreed initiatives to improve knowledge, skills and practice at supporting people who have a learning disability (i.e. champions network, training, reasonable adjustments)
You will attend meetings as agreed with the Lead Nurse to promote access for people who have a learning disability and / or autistic people (i.e. promoting reasonable adjustments, hospital passports, champions, etc.)
You will develop the skills of students and junior staff.
You will maintain a visible profile within the service (i.e. involvement in promotional materials, activities and to encourage liaison from acute trust services)
You will contribute to and deliver training as required to acute hospital staff.

- Registered Health Care Professional (LD Nurse, Adult Nurse, Mental Health Nurse, OT, S<, etc.) and current registration.
- Demonstrable Post registration development in areas relevant to this role

- Post-graduate qualification in relevant professional area (i.e. Epilepsy, autism, PBS, management)
- Qualification in teaching / assessing.
Skills, Knowledge & Experience

- Experience of providing professional healthcare interventions to influence and improve the care of people who have a learning disability / autism accessing health services
- Knowledge of the range of challenges experienced by people who have a learning disability / autism accessing acute services and the reasonable adjustments to support them
- Experience and knowledge of meeting the needs of autistic people

- Clinical skills across a range of professional interventions (i.e. different health needs and different settings)
- Knowledge of acute hospital systems and procedures
- Experience of audit and evaluation of systems of care
