The Sirona Adult Learning Disability Health Service (ALDHS): 

The ALDHS is a centre of excellence for specialist learning disability health needs. We are organised in 3 localities where multidisciplinary teams enable our service users to access health services through direct care, liaison with other services and offering support and training on how to work with people who have a learning disability (LD) and how to make reasonable adjustments.  

We are looking for a Clinical Specialist Community Disability Nurse for our community learning disability nursing team to support the Clinical Lead Learning Disability Community Nurse with day to day Operational management and co ordination of resources making sure that the service is responsive to the needs of the service users and their Carers and to the changing demands of the local health community

To provide Learning Disability specific advice and support to the other staff in the SALDHS and wider Sirona teams as necessary.

The successful applicant will provide clinical and managerial supervision of the ALDH community nursing staff and contribute to a matrix supervision approach for all staff within the ALDH service. This will also include responsibility for directly supporting the care and treatment of service users, carrying a small complex clinical caseload for those requiring a high level of expertise and experience.

 

 

Main duties of the job

  • Provide leadership, clinical support, and supervision to motivate team members and develop effective team working and personal development.
  • To triage referrals & be a key point of contact for the SALDHS.
  • Undertake highly skilled and specialist holistic assessment of service users to formulate individualised management and treatment plans using evidence based clinical reasoning skills and utilising a wide range of treatment skills and options.
  • Assess, develop, implement, and evaluate evidence-based programmes of care for individuals with complex needs who have a learning disability referring to other services or professionals as appropriate.
  • Ensure that service users, and if appropriate their caregivers and family, have access to the information they need in order to make informed choices about their care and promote shared decision making with individuals regarding their care plan. Encourage independence and enable service users and their carers to manage their condition
  • Act as expert resource, providing specialist LD Community Nursing information and advice to other community teams, their staff regarding the management and treatment of people with a learning disability who have complex health needs
  • Have a comprehensive knowledge of other agencies and services available, and how to access them appropriately.

Essential criteria
  • Diploma/ degree in LD Community Nursing
  • Evidence of study at Masters level or willingness to undertake
  • To be able to travel between a variety of locations within BNSSG. A motorised vehicle is required.
  • NMC Registration

Skills Experience and Knowledge

Essential criteria
  • Evidence of highly competent clinical reasoning skills and be able to communicate this to other professionals
  • A high level of effective communication and an ability to develop relationships with people with Learning disabilities, carers and staff in other teams and organisations.
  • Able to work collaboratively within own and multidisciplinary health and social care teams
  • Evidence of Computer skills including e-mail, word processing, power-point and the use of data bases to search for literature.
  • Demonstrable people management skills: you will need to be able to supervise and manage junior staff and students.
  • Documented evidence of CPD in accordance with NMC recommendations
  • Significant experience of the core areas of LD Community Nursing
