Are you ready to take on a strategic leadership role in community-based care? We’re seeking someone with proven expertise and a commitment to clinical excellence to help lead and shape the NHS@Home service across Bristol, North Somerset and South Gloucestershire (BNSSG).

This is a key leadership position, providing strong, visible and expert clinical guidance to support the planning, delivery and continuous improvement of the NHS@Home pathway. You’ll be working collaboratively with a dedicated team, including the Clinical and Operational Lead (CLOP), Operational Hub Manager, Advanced Clinical Leads (ACLs), and the Business Support Coordinator (BSC).

Your clinical insight will be vital in maintaining high standards of care and driving clinical excellence. You’ll play a central role in upholding clinical effectiveness, efficiency, and supervision across the service—ensuring that every decision and development is grounded in quality care.

Main duties of the job

  • To be responsible for the clinical management of the team and to provide direct and indirect line management. 
  • This post will focus on clinical quality, service improvement, clinical governance & innovation, clinical incidents & risk management within the service.
  • As clinical manager, the post holder will provide highly level assessment and treatment plans for patients within NHS@Home and will also provide clinical support to staff within the team and wider community to manage patients effectively.

Person specification

Qualifications & Training

Essential criteria
  • Current NMC Registration
  • Current knowledge of acute community management, national and local guidelines and evidence base via current research and journals
Desirable criteria
  • Masters level clinical qualification or evidence of working at masters level.
  • Coaching / Teaching and Assessing qualification / experience.
  • Knowledge of local, national, strategic and operational policy developments and an awareness of their implications.

Experience & Knowledge

Essential criteria
  • Experience of initiating, implementing and managing change and service improvement
  • Experience of caseload management and care planning
  • Knowledge of NHS@Home National Guidance
  • Experience of providing clinical leadership
  • Excellent communication, negotiation and influencing skills
  • Have and maintain the ability to undertake clinical practice at an expert level
  • Experience of managing complex care within Primary care/ secondary/ community care
  • Flexibility with working locations across BNSSG
