Job overview
We have an exciting opportunity for a highly motivated nurse with excellent communication skills to join our team as a Senior Staff Nurse on our Sub-Acute Stroke Unit.
Our unit consists of 30 beds at South Bristol Community Hospital, which provides a purpose built rehabilitation environment where individuals who are ‘medically optimised’ can receive rehabilitation assessment, along with focused therapy and support from our multidisciplinary team, to support independence.
Working closely with the Unit Manager and alongside our supportive Band 6 team, you’ll play a key role in the smooth running of the unit, while developing your leadership skills and building your expertise in the rewarding and evolving field of Stroke Nursing.

Main duties of the job
- Responsible for the initial holistic assessment , implementation and evaluation
of programmes of care for service users who are acutely and chronically
unwell; often with complex needs arising from the stroke and frail elderly
cohort - Use clinical reasoning skills to assess, plan, implement and evaluate, person-
centred intervention programmes incorporating physical, psychological, social,
leisure and employment needs, using an evidence based practice approach,
promoting high quality and harm free care - Collaborate and negotiate with patients, carers and family in identifying appropriate
and achievable goals as part of the overall care plan, which will maximise quality
of life and independence. - Facilitate appropriate referrals to other services or investigations based on your
assessment to ensure holistic patient care - Work within a multi-disciplinary team, to provide integrated skills and sharing
knowledge providing holistic and specialist skills supported by a robust
competency framework - Carry out nursing intervention drawing on a wide range of specialist knowledge
gained from practical experience and specific training

Desirable criteria
- Recent relevant experience within the community setting including management of the frail complex patient
- Experience of supervising staff and students from other professions
