We are seeking a Nurse or Paramedic to join our team!

NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly, to.

The service uses a mixture of face-to-face visits & telephone  calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.

NHS@Home has pathways, covering a wide range of conditions including frailty, respiratory, heart failure, OPAT, Infection and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.

The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust.

Prif ddyletswyddau'r swydd

Your duties will require exemplary clinical, communication, IT and trouble shooting skills to enable you to:

  • Provide clinical face to face and virtual  monitoring and assessment of patient in the @home service, advising on care and treatment and reporting changes in patient condition as required in line with local protocols and guidance. 
  • Providing acute care to patients (in their homes) with complex and specialist needs in the community, across Bristol, South Gloucestershire and North Somerset.  
  • Working alongside the multidisciplinary team to develop, implement and evaluate care plans.  
  • Assessing the health and psychosocial needs of the patient within the hospital and community setting.  
  • Plan and implement treatment and use a wide range of service to meet their needs.  
  • Utilising a wide range of clinical skills to support the ongoing management of patients in their home environment e.g. Comprehensive Health Assessment, observations, venepuncture, ECG, bladder scanning and wound care.  
  • Communicating effectively, networking and liaising to ensure high quality responsive, safe, efficient and compassionate care is provided for our patients, carers and families.

  • Ability to carry out complex health assessments, including: Venepuncture, Observations, Bladder scanning, ECG & wound care
  • Ability and evidence to use Microsoft packages including Outlook, MS Teams, Excel and electronic patient records
  • Experience & ability to work autonomously
  • Experience & ability to understand knowledge base and to escalate if needed
  • Evidence of continuous personal/professional development
  • Adaptability and Flexibility for effective time management within conflicting priorities
