Job overview
This is a fantastic opportunity to join a supportive community team, helping people rebuild their lives after stroke by working towards goals that truly matter to them.
In this role, you’ll provide assessment and intervention to support individuals both as they transition out of hospital and throughout their longer-term rehabilitation journey. You’ll work as part of a multidisciplinary team, offering excellent opportunities for learning, development and regular clinical supervision.
You will take clinical responsibility for a caseload of stroke patients with complex needs, using your expertise to assess, plan, implement and review evidence-based, person-centred care. The role requires strong organisational skills, with the ability to manage and prioritise your workload effectively, including responding to changing and urgent clinical needs.
A key part of the role is empowering people to manage their condition confidently and independently—helping to prevent complications, maximise recovery, and improve overall quality of life.

Main duties of the job
- To provide specialist assessment and intervention for people after stroke in their homes and local community
- To hold a caseload of patients with complex needs and organise this effectively
- To be involved in monitoring and developing the stroke service along with senior colleagues
- Contribute to the training and supervision of junior staff and students
- To work holistically as part of the MDT, across the stroke pathway and with other community services
