Job overview
An exciting opportunity has arisen to join the newly established Burren Intermediate Care Beds Service as a Band 5 Physiotherapist. This innovative service forms a key part of the BNSSG discharge and patient flow pathway, supporting individuals who no longer require acute hospital care but continue to require rehabilitation, recovery support and multidisciplinary intervention before returning home.
The service currently provides 32 intermediate care beds alongside a specialist bariatric pathway and is designed to support timely transfers of care, maximise patient independence and improve outcomes through high-quality rehabilitation.
As part of a dynamic multidisciplinary team, you will work closely with Occupational Therapists, Nursing colleagues, Medical staff, Social Care professionals and Community Services to deliver person-centred rehabilitation programmes that enable people to achieve their goals and return to their preferred place of residence.
This is a unique opportunity to join a developing service during an exciting period of growth and transformation, helping to shape rehabilitation practice and contribute to a model that supports patient flow across the wider health and care system.
Main duties of the job
- To provide a safe, effective, patient-centred and evidence-based Physiotherapy service within the Intermediate Care Beds Service.
- To assess, treat and manage patients requiring rehabilitation, recovery and discharge
planning support. - To work as an autonomous practitioner managing a defined caseload within professional scope of practice.
- To contribute to person-centred rehabilitation programmes that maximise independence and support positive outcomes.
- To work collaboratively within the multidisciplinary team to facilitate safe and timely discharge planning.
- To promote Home First principles and support individuals to achieve their rehabilitation goals.

Qualifications
Essential criteria
- BSc (or equivalent recognised qualification) in Physiotherapy.
- Current HCPC registration.
Desirable criteria
- Membership of the Chartered Society of Physiotherapy.
- Up to date knowledge of Evidence Based Practice, local and national standards
Skills, Knowledge and Experience
Essential criteria
- Competent assessment and treatment skills including manual therapy, exercise prescription, directing a class
- Able to make clinically reasoned judgements when working with a variety of conditions
- Experience of working within a multidisciplinary team
- Able to adapt communication for individuals with communication difficulties
Desirable criteria
- Experience of community, community inpatient or intermediate care rehabilitation settings
