Job overview

NHS@Home is an innovative service, providing care for people who are acutely unwell in their own homes across Bristol, North Somerset, and South Gloucestershire. It offers an alternative to hospital and helps people return home from hospital more quickly, too.

The service uses a mixture of face-to-face visits, phone and video calls, and remote monitoring through digital technology, to help people recover safely in the comfort of their own home.

NHS@Home has six pathways, covering a wide range of conditions including frailty, respiratory, heart failure, and general surgical recovery. Our team provide a very wide range of acute healthcare treatments within the home, in addition to monitoring health conditions and vital signs for deterioration.

The service is run in partnership between Sirona care & health, North Bristol Trust, University Hospitals Bristol and Weston NHS Foundation Trust and BrisDoc.

"Knowing I was being discharged into the care of the NHS@Home respiratory service made the world of difference to me - I felt very reassured by the fact that the respiratory team were monitoring me so frequently it helped me feel safer and less afraid." -N.L, service user
