An exciting opportunity has arisen for a Band 6  Clinician to join the NHS @ Home respiratory pathway. You'll be based within the Sirona  NHS@Home team and work alongside the Community Respiratory Team to support patients referred to the NHS@Home virtual ward.

We are looking for a highly skilled and experienced clinician that can deliver high quality care to patients referred to the acute pathway. 

Extensive experience in respiratory management isn't essential but a keen interest to learn and develop in the role is a must. 

This role is open to Registered Nurses, Physiotherapists  and Paramedics. 

We are an innovative team whose ethos is working collaboratively to deliver patient centered care. This post offers a high level of professional support, with a strong focus on Professional Development.

The NHS@ home Respiratory pathway provides care across Bristol, South Gloucestershire and North Somerset therefore you must be adaptable to the needs of the service and staff across the BNSSG area. The service  currently provides a 7-day service 8am-8pm including weekends and bank holiday cover.

Main duties of the job

  • Undertake expert specialist respiratory assessment of services users, including those with complex presentation, to formulate individualised management and treatment plans using evidence based clinical reasoning skills and utilising a wide range of treatment skills and options.

  • To perform specialist respiratory assessments by undertaking a holistic approach and following local and national guidelines and protocols.

  • Work with service users and their carers towards shared goals providing advice and support, signposting to other community services and organisations to ensure person centred care and promoting physical, psychological and social wellbeing.

  • Provide education and advice to enable self-management and prevent unnecessary hospital admissions.

  • Be a proactive member of the multi-disciplinary team and have a significant role in the on-going service development, delivery and evaluation of the Respiratory NHS@Home service across BNSSG.

Qualifications

Essential criteria
  • Current registration with NMC or HCPC
  • Evidence of continuous personal and professional development and ability to study at Masters level.
Desirable criteria
  • Qualifications/study in respiratory care
  • Student mentorship/ practice assessor qualification
  • Physical assessment and clinical reasoning qualification

Experience

Essential criteria
  • Experience of managing and reviewing the chronic respiratory cohort and demonstrable evidence of having worked recently with patients with respiratory disease.
  • To be competent in performing specialist clinical assessments and treatments with a variety of presenting conditions, and with particular emphasis on respiratory examination, treatment techniques and clinical reasoning
  • Experience of community and multidisciplinary team working
  • Excellent communication, negotiation and influencing skills
  • Experience of managing and leading people
  • Previous post qualification experience working in a health and social care environment within the relevant professional discipline
Desirable criteria
  • Experience of providing assessment and treatment plans via telephone or other virtual means
