Following a hospital stay, the ethos across Bristol, North Somerset and South Gloucestershire (BNSSG) for all people being discharged is ‘Home is Best’, for some service users who have complex needs or need a period of further assessment this is not practicable and they transfer to a complex assessment bed which is generally in a care home.

During their stay, service users will be given support, along with their carers and families around complex discharge planning. This will enable a timely and safe discharge to the person’s usual residence or to a more suitable location if appropriate.

We actively promote a multi-disciplinary model where staff work closely with social workers, GPs as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision.

Main duties of the job

  • Promote optimum independence through encouraging rehabilitation techniques

  • As a rehab support worker you will be a key figure in completing the day to day rehabilitation with patients under the instruction of the lead therapists. You will be required to provide feedback on the service users’ progress and help to ensure that the service users achieve their maximum potential.

  • Undertake a range of delegated clinical and therapy led activities to support the service users on the caseload.

  • The post holder will work under direct supervision and be responsible to senior therapists providing one to one and group rehabilitation.

  • You will work within the Adult Service to deliver a full range of health and social care activities, both acute and long term, supported by a robust competency framework to adults within the complex assessment beds.

  • You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, physiotherapists, occupational therapists and other Allied Healthcare Professionals

  • The Service operational Hours are between 08:00 and 18:00 (flexible working can be discussed) with future possibility for weekend cover.

  • In order to provide organisational and service resilience, you may be expected to support other teams or services, both within the locality and Sirona wide when the need arises at short notice working on a rota basis

  • NVQ level 3 in Health & Social Care or willingness to work towards
  • Numeracy and literacy level 2 or equivalent
Desirable criteria
  • Apprenticeship in Health level 3

Skills, Knowledge & Experience

Essential criteria
  • Ability to work as part of a multidisciplinary team but to also act on own initiative
  • Effective communication and interpersonal skills including the ability to use tact and persuasive skills where necessary
  • Experience of direct patient care in a health or social care setting
  • Able to make correct decision within a set of clear guidelines
  • Able to use a variety of IT systems e.g. email, EMIS
