Job overview
This is an exciting time to join the Primary Care Team in Flintshire which is undergoing transformation. These Primary Care Practitioner posts are integral within the Tier 1 Primary Care Mental Health Community Service within Flintshire. The existing team include qualified practitioners including Mental Health Nurses and Social Workers, Star workers and Psychology who are all delivering evidenced based interventions. We are seeking enthusiastic, compassionate qualified practitioners to join us in delivering the best possible assessments and interventions. The post holder will be required to work within a multi-disciplinary team and in collaboration with third sector services.
Main duties of the job
The post holder will undertake detailed assessments and hold a caseload providing short term evidenced based interventions for 6 to 10 weeks. To work in collaboration with third sector services and refer people onto services within their communities. To provide supervision, guidance and support to junior members of staff.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
