The post holder is responsible for the assessment, planning, implementation and evaluation of evidenced-based nursing care, working collaboratively and co-operatively with others to meet the needs of patients and their families.
Assist in the management and organisation of their clinical area and participating in the education, development and supervision of other staff members.
Main duties of the job
To assist the Primary Care Treatment Room service and other care services;
- Assess the patients nursing needs, plan, implement and evaluate nursing care in order to ensure the delivery of effective patient care.
- Implement and evaluate individual treatment plans for patients with new and known long-term conditions in relation to the care that is provided/required within the Treatment Room Unit.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective urgent and emergency care.
- Communicate complex and sensitive information to patients/families, including patients with special needs/learning disabilities or other barriers to communication.
- Follow and adhere to national local guidelines and policies.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications and Knowledge
Essential criteria
- NMC Registered with active UK status
- Knowledge of needs of patients with short and long-term conditions
Desirable criteria
- Evidence of post registration study and training e.g. POVA, Child protection, ILS
- Previous experience using EMIS, Oracle, Formeo and WCP
Experience
Essential criteria
- Pre-registration evidence in direct nursing care
- Experience of providing holistic nursing care
Desirable criteria
- Implement evidence based practice
- Community/Primary Care nursing experience, in particular wound care
