Job overview
The post holder is required to work within an MDT focusing on the older person living in the community who suffer with a wide range of mental health problems.
Main duties of the job
The post holder will be responsible for assessing client’s needs, planning and implementing the care package alongside other professionals within the team.
Post holder will provide education, knowledge, guidance and support to student nurses and unqualified staff in the team and provide support to clients and their families.

Detailed job description and main responsibilities
In conjunction with other team members, to take on the role of care coordinator for a designated caseload of clients with both organic and functional mental health problems.
To be responsible, for the comprehensive assessment, planning, implementation of programmes of care, providing each client with an individual holistic package of care. To ensure Mental Health Measure and care management protocols are adhered to and standards are met.
As care coordinator, responsibility for making evidence based assessment of client’s physical, psychological, spiritual and social needs taking into account and respecting ethnicity, for a defined caseload.

Qualifications
Essential criteria
- • RMN • Educated to Diploma Level or equivalent
Desirable criteria
- • Broad range of experience working in health & social care settings
Skills
Essential criteria
- Ability to prioritise and work proactively Ability to manage a designated caseload without direct supervision
Desirable criteria
- IT skills Phlebotomy skills Monitoring clients physical observations
Personal qualities

- Ability to communicate clearly and accurately Ability to adapt to new and changing circumstances Self-motivated

Knowledge
Essential criteria
- Demonstrate a good understanding of mental health problems in the older person. Awareness of current developments within a clinical setting. Completed training in mentoring & Assessing student nurses
Desirable criteria
- Awareness of current developments within MHLD older peoples service Understanding of clinical governance

You must have appropriate UK professional registration.
