Cancer Pathway Co-ordinator- NSS and Skin

An exciting opportunity has arisen in the Cancer Services Team for a Cancer Patient Pathway Co-ordinator covering NSS and Skin pathways.
By monitoring and tracking cancer patients from the point of referral, and reporting on cancer waiting times, you will play a central role in ensuring their timely diagnosis and treatment. We are looking for a flexible and self-motivated administrator with good attention to detail and the initiative to work independently. This is a busy and demanding role requiring a high level of communication and organisational skills.
The role requires developing excellent working relationships with many key stakeholders such as Consultants, Clinical Nurse Specialists and MDT Co-ordinators at other Trusts.
An ability to meet deadlines and awareness of confidentiality issues are essential.
Good levels of computer literacy are essential, including previous experience of computerised information systems and experience of MS Office packages. It is also essential that the postholder has previous NHS experience.
Main duties of the job
The post holder will be responsible for:
- Prospective and proactive management of the inter-trust cancer pathway, monitoring, reporting and tracking cancer patients, ensuring that the patient’s journey is processed in a timely and efficient manner, in line with the SSG agreed pathway and with the Cancer Waiting Times Targets.
- Resolving delays in the pathway at the earliest opportunity and where this is not possible escalating issues in accordance with Trust and network procedures
- Co-ordinating and providing administrative support to the cancer S/LMDT(s), ensuring that all relevant paperwork and clinical information is available including case notes, imaging and pathology as necessary. To attend S/MDT’S keeping comprehensive records of attendance, outcomes as discussed.
- Ensuring the timely and efficient transfer of patients and information between trusts involved in the patient pathway, adhering to network SSG agreed communication pathways and protocols
Data collection and analysis:
- Ensuring data collection is undertaken in a prospective manner, assuring accuracy at all times. Using both local and national datasets, including Open Exeter and the Trust Cancer waiting Times data base.
- Analysing target breaches and report to Cancer Manager and Lead S/LMDT clinician and S/LMDT Service Improvement Clinical Lead

Detailed job description and main responsibilities
1.1 GENERAL DUTIES AND RESPONSIBILITIES
1.1.1 General administrative duties for the department such as faxing, filing of all correspondence (including patient letters until EDRM introduced), results, photocopying and distribution of mail - identify and action any correspondence suitable for own response - produce relevant correspondence for consultant's reply.
1.1.2 Ensure copies of letters, discharge summaries etc. are sent, on request, within the agreed timeframe where appropriate.
1.1.3 Support the team in answering telephone calls and dealing with enquiries in a helpful and courteous manner, using initiative and discretion or directing the call to the appropriate staff member.
1.1.4 Ensure requests from clinicians/nursing staff for clinic changes, cancellations and creations are processed with the appropriate team in accordance to trust policy.
1.1.5 Respond to requests from clinicians to type up urgent letters when required.
1.1.6 Ensure all patient demographics and contact details are kept up to date on PiMS/Somerset.
1.1.7 When accessing patient records ensure computerised tracking systems are used appropriately.
1.1.8 Phone patients to provide instructions, reminders and information where necessary.
1.1.9 Attend and participate in meetings relevant to the role as required.
1.1.10 Check with Admissions on a regular basis that patients have been added appropriately to the elective waiting list.
1.1.11 Provide reasonable cross-cover for other patient navigators to ensure a consistent and effective administration service is maintained at all times.
1.1.12 Process confidential patient-sensitive documentation to and from patients, other healthcare professionals and external agencies.
1.1.13 Deal with concerns/complaints as appropriate, immediately escalating where necessary to the appropriate manager for prompt action.

1.2. PATIENT PATHWAY PLANNING
1.2.1 Manage referrals received by the specialty by working closely with clinicians to ensure that patients are placed on the correct clinical pathway.
1.2.2 Ensure that simple tests or pre-investigations have been ordered where necessary.
1.2.3 Ensure effective communication channels are in place and that key stakeholders including patients, families and carers are informed of the communication details and hours of work.
1.2.4 Arrange transport (in line with trust procedure) and translation services where necessary.
1.2.5 Identify and seek to resolve bottlenecks or delays in the patients’ pathways, escalating issues where necessary to the relevant service manager. Ensure that all patient administration is carried out in line with Trust policies and procedures.
1.2.6 Coordinate appropriate appointments for patients in partnership with the booking Office/RAS Team as directed by the clinicians in line with the clinical pathway and the patient’s individual needs to prevent multiple hospital attendances.
1.2.7 Monitor clinic lists to assess workflow and escalate any issues where necessary.
1.2.8 Ensure the patient understands their care pathway and is aware of appointments and any changes.
1.2.9 Monitor investigations and tests to ensure they have been completed and results/reports are available prior to appointments and consultations with the clinical team.
1.2.10 Ensure all appointments are compliant with the Trust Access Policy and national targets. Where appointments are cancelled these should be rebooked within the appropriate time frame.
1.3. RESPOSIBILITY FOR INFORMATION AND RESOURCES
1.3.1 Notify relevant departments and Medical Records when informed of the death of a patient and cancel future appointments to prevent further distress for families.
1.3.2 Ensure a high standard of data quality is maintained, arranging audit support and data validation where appropriate. This may include the coding of outpatient procedures as required.
1.3.3 Support the team as required with updating data on relevant shared drives and departmental databases.
1.4. PROFESSIONAL AND PERSONAL DEVELOPMENT
1.4.1 Attend mandatory training required for administrative staff as outlined in the Trust Mandatory Training Framework and any training where updating of skills/knowledge is required.
1.4.2 Be proficient in the use of all electronic systems in relation to patient data and undertake training as required if new technology/electronic systems are introduced, to ensure competencies are maintained and efficiencies of the administrative processes maximised.
1.4.3 Participate in yearly staff appraisal, identifying professional development objectives and learning requirements.
