Paediatric Diabetes Family Support worker

Job overview

We’re looking for a motivated individual to join our Paediatric Diabetes Team, supporting children and young people across hospital and community settings. You’ll play a key role in delivering specialist care, helping families implement and manage diabetes technology, and providing hands-on support in clinics, schools, and homes.

Strong IT skills and experience in paediatric care are essential, along with the ability to communicate effectively—especially in languages spoken within the local community. You’ll assist with device downloads, troubleshoot tech issues, and help set up drop-in clinics. Your work will also involve supporting emotional and social development through therapeutic activities.

This role requires excellent communication, sound judgement, and the ability to work across professional boundaries to educate and empower families throughout their diabetes journey.

Main duties of the job

The post holder will work closely with children and young people, helping them actively engage with their diabetes care. This role involves working across both hospital and community settings, offering personalised support to patients and their families.

A key part of the role is to motivate and empower children, parents, and carers to improve their health and wellbeing. The ability to speak languages commonly used in the local community is essential to ensure clear and inclusive communication.

In clinic, the post holder will assist with downloading insulin pumps and other diabetes technology, providing timely feedback on any issues or concerns to support effective care and decision-making.

Detailed job description and main responsibilities

The post holder will use their IT skills to support families in resolving technical issues related to diabetes technology. They will take the lead in setting up drop-in clinics for downloading insulin pumps and glucose sensors, ensuring accessible and timely support.

They will also work closely with schools to enable the effective use of diabetes technology for children and young people during the school day. Additionally, they will be responsible for ordering and managing equipment used for teaching and demonstrating technology, helping families feel confident and informed.

Qualifications

Essential criteria
  • A levels/ Diploma in health or NVQ in health and social care/ Nursery nurse
Desirable criteria
  • Successful completion of foundation degree equivalent to diploma level
  • IT qualification

Experience

Essential criteria
  • Demonstrable experience of working with children, young people, families and wider support networks.
  • Experience of working with CYP in various settings.
Desirable criteria
  • Experience of working with children and young people with diabetes
  • Experience of working within a multi-disciplinary team.
  • Experience of working in a community role
  • Experience of teaching within group settings.

Skills

Essential criteria
  • To be able to communicate effectively with the team, families and children and young people within the caseload.
  • Evidence of effective inter-personal communication skills.
  • Development of supportive relationships with patients, carers and other professionals
  • Ability to work effectively as part of a multi-disciplinary team.
Desirable criteria
  • Clinical skills e.g. measurement and documentation of height weight, blood pressure, urinalysis and HbA1c
  • Ability to coordinate and work within group settings
  • To be multilingual
