Job overview

We are looking for a motivated Nurse to join our Paediatric Diabetes Team. The individual should have experience in caring for children and young people with diabetes, and be able to work in both Hospital and community settings. Applicants should have knowledge and experience of insulins and devices, have an insight into teaching newly diagnosed patients and have an awareness of blood glucose sensors and insulin pump therapy.   The post requires you to act as a key resource in facilitating and supporting the delivery of direct specialist patient care.   

Applicants should have good knowledge of DKA and illness in childhood diabetes and also have the ability to complete a comprehensive clinical assessment. The applicants will be able to work across medical/nursing professional boundaries to provide education and support to the child and family throughout their diabetes journey.  Applicants must be able to demonstrate complex communication skills, judgement, discretion and decision making with the child and family and appropriate professional agencies where necessary. 

 

Main duties of the job

Perform comprehensive specialist assessment of individual patient needs.

Plan, implement and evaluate the complex care delivered for children in primary and secondary care settings throughout childhood, to ensure a smooth transition to adult services.  

To be involved in teaching programmes for medical and nursing colleagues in the management of diabetes, including insulin pump therapy.

Communicate information sensitively with parents and children, regarding life-long condition, disease management and prognosis.

Detailed job description and main responsibilities

 To act as a key resource in facilitating and supporting the delivery of direct specialist patient care to all members of the multidisciplinary team, eg, nurses, doctors, teachers, school and community nurses.

To monitor the treatment and care of all paediatric diabetic patients in conjunction with medical and nursing staff as necessary. In particular, to assess, plan, implement, evaluate care for children with diabetes and support and educate all diabetic patients under the Consultant Paediatrician who are newly diagnosed or who are admitted to hospital with a glucose emergency or needing a surgical procedure; and all those who are having problems with their diabetes management as outpatients.

With Consultant or PDSN support, to be responsible for the paediatric diabetic caseload and be involved in service development within the designated area of diabetes.

To document prescriptions in patient’s medical notes during hospital stay and hospital diabetes database.

Qualifications

Essential criteria
  • RN Child or RSCN
Desirable criteria
  • Degree in diabetes management and willingness to attend diabetes management course

Experience

Essential criteria
  • current interest and experience in diabetes nursing
  • expert in communicating with children, family and multi-disciplinary team.
  • effective documentation skills
  • minimum 2 years experience post registration in children's nursing
Desirable criteria
  • familiar with audit, research and evidence based practice
  • knowledge of insulin pump therapy and evices

Knowledge
