Job overview

HSSIB operates under the Health and Care Act 2022 to improve patient safety across England through independent investigations. We work collaboratively across the NHS and wider system to share learning and strengthen investigation capability. We are an independent organisation focused on improving patient safety through expert, systems-based investigations, our reports do not apportion blame or liability; our work identifies learning to make healthcare safer for patients, families and staff.

We have recently launched our new strategy to help us build investigation excellence across healthcare and strengthen the capability of healthcare investigations. We are seeking an experienced Safety Investigator (Trainer) to join our team. This is a unique role combining high-quality investigation with hands-on training to build capability across the healthcare system.

Our team have diverse experience of healthcare and other safety critical industries, bringing new perspectives and a unique mix of clinical and non-clinical input to every aspect of safety investigation. As a Safety Investigator (Trainer), the post holder will work as part of this dynamic team delivering an effective service within a multi-disciplinary Investigations function facilitating, promoting and publicising improvements in the safety of care provision through systems-based investigation methodologies.

Main duties of the job

The core functions of the Safety Investigator (Trainer) are to:
●    Conduct investigations and produce reports that patients, families, carers and staff value, trust and respect, generating findings and recommendations that improve patient safety
●    Act as an exemplar for high-quality investigation across healthcare provision in England, combining expert patient safety investigation with hands-on training to strengthen investigation capability across healthcare organisations
●    Build local capability through real-time learning and modelling of best practice, supporting the development of accessible learning resources from investigation findings and tailoring approaches to sector-specific contexts
●    Facilitate work-based learning and apply adult learning principles to build sustainable investigation capability throughout the healthcare system

Detailed job description and main responsibilities

●    Undertake and complete patient safety investigations from initiation through to compilation and presentation of reports, findings, recommendations and observations, using systems-based methodologies.
●    Produce high quality investigation reports and recommendations that are accurate, comprehensive, well researched, and effective in reducing the likelihood of a reoccurrence of the event
●    Understand and abide by HSSIBs powers as detailed in the Health and Care Act 2022
●    Manage assigned investigations on a day-to-day basis, ensuring investigation guidance is followed consistently and that investigations are completed and published in a timely manner, supporting the Senior Safety Investigator (Trainer) with planning and scheduling. 
●    Effectively manage a range of investigations and projects to ensure effective delivery against key performance indicators
●    Gather and distill insights and intelligence to support identification of investigation topics and share knowledge to enhance standards of incident investigation across healthcare.
●    Engage in innovation and research relevant to healthcare safety investigations 
●    Provide training, facilitating real-time learning and modelling investigation best practice whilst adapting to sector-specific contexts.
●    Deliver training programmes that integrate investigation methodology with human factors and systems thinking.
●    Deliver technical consultation on investigation methodology, observing practice and providing constructive feedback.
●    Support the development of case studies and translate investigation findings into accessible learning resources; synthesise complex investigation findings into actionable learning and present insights to stakeholders.
●    Facilitate work-based learning and apply adult learning principles to build sustainable capability.
●    Actively participate in evaluation programmes to support the ‘Building Investigation Excellence’ strategy
●    Contribute to the development of investigation frameworks, best practice guidance, and professional standards.
●    Contribute to the development of investigation processes that support the principles of lifelong learning, ensuring lessons learned and debriefs are captured to enable continual improvements in the quality of investigation. 
●    Work with highly complex, sensitive and emotive information requiring analysis and interpretation to inform sound decision making, including working with patients, carers and relatives who have been harmed or bereaved and with affected healthcare professionals.
●    Engage patients and families in investigation processes, ensuring their perspectives inform both investigations and capability building. 
●    Support report publication and proactive dissemination of investigation learning.
●    Analyse, interpret and present highly complex and sensitive data to highlight issues, risks and support decision making, ensuring data gathered in the process of investigation is managed, stored and disposed of in accordance with information governance requirements.
●    Present complex sensitive or contentious information to different audiences
●    Support the management of third party expert staff contracted to work on specific investigations, and contribute to training staff in good investigation practice in healthcare organisations.

Person specification

Qualifications

Essential criteria
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent
  • Demonstrable postgraduate experience as a safety investigator, healthcare clinician or within a training and educational role.

Knowledge and Experience

Essential criteria
  • Clear understanding of human factors principles, from a causal and improvement science perspective
  • Demonstratable experience in delivering training, coaching or practical learning

Skills and Abilities

Essential criteria
  • Excellent planning and organisation skills with the ability to prioritise tasks for optimal personal and team performance
  • Ability to translate patient safety information and investigation findings into informed intelligence, and present this transparently, accurately, and clearly
  • Ability to Identify and translate learning into materials for sharing to improve patient safety
