Job overview

This 6 to 12 month post offers candidates the opportunity to gain experience in Intensive Care Medicine in a busy district general hospital whilst training in critical care echo gaining competency in Focused Intensive Care Echocardiography (FICE) and BSE level 1 echocardiography. There may also be opportunity to develop point of care US skills.

This job would be ideally suited to individuals who are committed to a career in Intensive Care Medicine (ICM) whether accrediting singly or with another specialty. Some experience in ICM is strongly desirable (for example a stage 2 ICM trainee looking to complete an out-of-programme year or special studies module.)

Main duties of the job

The clinical training programme is outcome-based and follows the curriculum developed by the Faculty of Intensive care medicine (FICM).  This is described in detail on the FICM’s documents on training and assessment (http://www.ficm.ac.uk). The echocardiography competencies are based on the Intensive Care Society’s recommendations.

www.ics.ac.uk/ics-homepage/.../focused-intensive-care-echo-fice/; http://www.ics.ac.uk/ics-homepage/accreditation-modules/cusic-accreditation/)

Clinical and Educational supervision will be provided by Dr Jodie Smythe, clinical lead for ICM and also level 2 BSE qualified. Within the ICU consultant body we also have a range of FICE mentors and an evolving POCUS group centred around Dr Andrew Walden who has an interest in point of care US and runs regular training courses.

Working for our organisation

The prospective candidate would work 70% of their time on the ICM rota and 30% of their time training in Ultrasound and Echocardiography. The candidate would accredit in FICE and CUSIC and would have the opportunity to develop other more advanced Echocardiography and Ultrasound skills. Specifically the option of working towards the full British Society of Echocardiography (BSE) echo accreditation could be facilitated. There is also the option of gaining competence in venous sonography and abdominal ultrasound scanning. We have developed collaborations in ultrasound research with Dr Kirk-Bailey at the University of Surrey (pleural effusions in critical care PECC study) and Professor Tim Harris at Bart’s and the London (barriers to implementation of lung ultrasound amongst trainees)

Detailed job description and main responsibilities

We are able to provide advanced respiratory, cardiovascular and renal support to all our patients. We have a national reputation for percutaneous tracheostomy insertion (and are able to train in this domain); renal replacement techniques; bronchoscopy and mechanical ventilation. There is a 24 hour Critical Care Outreach Service and weekly ‘recovery after critical care’ clinic. Exposure to standardisation and processes of care will be encouraged. We have an advanced nurse practitioner training programme, which started in September 2017 and currently train 4 trainee advanced critical care practitioners.

We are a recognised regional centre for training in ultrasound and echocardiography. We have close links with the cardiology department and Dr Elkington, consultant cardiologist is one of our local BSE supervisors. Our department have trained a large cohort of trainees to complete FICE. Dr Walden helped develop the CUSIC competencies for the Intensive Care Society and chairs the Focused Acute Medicine Ultrasound (FAMUS) group. Both FICE and CUSIC courses are run three times a year in the trust education centre or local venues and are attended by delegates from around the country with excellent feedback
